Çocuk ve adölesanlarda sekiz yıllık testis torsiyonu deneyimlerimiz

Özet

Amaç: Testis torsiyonu, testis ve spermatik kordun ekseni etrafında dönüşü ile karakterize akut skrotal ağrıya neden olan patolojik bir durumdur. Çalışmamızda 8 yıllık dönemde kliniğimizde testis torsiyonu tanısıyla opere edilmiş 43 hasta retrospektif olarak incelenmiştir.Yöntem: Ocak 2008 - Aralık 2015 tarihleri arasında testis torsiyonu öntanısıyla opere edilen 43 hasta retrospektif olarak değerlendirildi. Bulgular: Testis torsiyonu öntanısıyla opere edilen hastaların 8'inde operasyonda testis torsiyonu saptanmamış, 35'inde testis torsiyonu saptanarak detorsiyon veya orşiektomi uygulanmıştır. Yaş aralığı 0-17 yaş arasında değişmektedir (ort.10.8). Hastaların 23'ünde (%69) sol, 12'sinde (%31) sağ testis torsiyonu saptanmıştır. İlk 12 saatte başvuran 13 (%37) hastanın hiçbirinde orşiektomi gerekmemiştir. On iki-yirmi dört saat arasında başvuran 18 hastanın (%52) 8'ine (%23) detorsiyon, 10'una (%29) orşiektomi gerekmiştir. Yirmi dört satten geç başvuran 4 hastamızın (%12) birine (%3) detorsiyon, diğer 3 hastaya (%9) orşiektomi yapılmıştır. Sonuç: Çocuk ve adölesanlarda testis torsiyonu akut skrotumun en sık nedenlerinden birisidir. Testis torsiyonuna yol açan en önemli neden erkeklerin %12'sinde saptanan bellclapper (çan tokmağı) deformitesidir. Eksplorasyonda testis kaybını belirlemede en önemli kriterler torsiyonun derecesi ve süresidir. Sonuç olarak, testis torsiyonu çocuk ve adölesanlarda en sık görülen ve testis kaybı ile sonuçlanabilen, en ciddi akut sktrotum nedenidir. Tedavide başarı oranını belirleyen en önemli etken ise erken tanı ve erken cerrahi uygulamadır.

Abstract

Objective: Testicular torsion is a pathologic condition characterised by the rotation of spermatic cord and testis around its axis which causes acute scrotal pain. In our study 43 patients were analyzed retrospectively which were operated in our clinic with a diagnosis of testicular torsion during the 8-year period.Methods: Forty-three patients operated with the initial diagnosis of testicular torsion between January 2008, and December 2015 were retrospectively evaluated.Results: We did not detect testicular torsion intraoperatively in 8 patients who were operated with the initial diagnosis of testicular torsion, while in 35 patients testicular torsion was detected and detorsion or orchiectomy was performed. Age range of the patients varied between 0, and 17 years. Twelve (31%) patients had right and 23 patients (69%) left testicular torsion. In thirteen patients (37%) presenting within the first 12 hours, orchiectomy was not required. Eighteen patients (52%) admitted between 12-24 hours, and 8 (23%) of them required detorsion and the remaining 10 (29%) cases necessitated orchiectomy. Four (%12) patients admitted later than 24 hours who underwent (n=1;3%) detorsion or orchiectomy (n= 3;9%).Conclusion: Testicular torsion is one of the most common causes of acute scrotum in children and adolescents. The bell-clapper deformity detected in 12% of males is the most important reason that leads to testicular torsion. In exploration, the most important criteria in determining the loss of testis is the degree and duration of torsion. As a result, testicular torsion is most common and serious cause in children and adolescents that may result in loss of the testicles. The most important factors that determine the success rate of treatment is early diagnosis and surgery.
