Akut ve Geçici Psikotik Bozukluk İzleminde Tanı Stabilitesi: Bir Ön Çalışma

Özet

Giriş: Akut ve geçici psikotik bozukluk diğer psikotik bozukluklardan akut başlangıcı, stresli yaşam olaylarını izlemesi ve kısa sürede tam düzelmenin görülmesiyle ayrılmaktadır. Tedaviye yanıtın ve gidişin iyi olduğu ileri sürülmektedir. Hastaların bir kısmında daha sonra tanı, şizofreni ya da duygudurum bozuklukları olarak değişmektedir . Çalışmamızda akut ve geçici psikotik bozukluk tanısı konan hastaların 5 yıllık izleminde ortaya çıkan tanı değişikliklerini, sosyodemografik ve klinik özelliklerini araştırmak amaçlanmıştır. Yöntem: Örneklemimiz 2010 yılında yatarak tedavi gören ve akut ve geçici psikotik bozukluk tanısı alan hastalardan oluşturulmuştur. 2015 yılında bu hastaların tıbbi kayıtları geriye dönük olarak 5 yılı kapsayacak şekilde incelenmiştir. Bulgular: Toplam 47 hastanın 17'si kadın, 30'u erkek, yaş ortalaması 32,2±9,9'du. Çoğunluğu bekar (%51,1), çalışmıyor (%85,1) ve ailesiyle yaşıyordu (%87,2). Hastaların %51,1'i tekrar yatarak tedavi görmüş, %8,5'inin ise tekrar başvurusu olmamıştı. ızlemde en sık konulan tanılar organik olmayan psikotik bozukluk (%38,3), şizofreni (%25,5) ve bipolar duygulanım bozukluğuydu (%12,8). 2 hastada alkol ya da madde kullanımına bağlı psikoz, 3 hastada nörolojik bir hastalık mevcuttu. Sonuç: Çalışmamızda akut ve geçici psikotik bozukluk tanısı alan hastaların çoğuna izlemde organik olmayan psikotik pozukluk ya da şizofreni tanısı konduğu saptanmıştır. Bu nedenle özellikle risk faktörlerini taşıyan akut ve geçici psikotik bozukluk olgularının şizofreninin prodromal dönemi açısından ayrıntılı değerlendirilmesi ve uzun dönem yakın izlemi faydalı olacaktır .

Abstract

Introduction: Acute and transient psychotic disorder differentiates from other psychotic disorders with acuteonset, occurrence after stressful life events and endingwith full remission. It is suggested to have good prognosis and response to treatment. In some of thepatients, the diagnosis is switched to schizophrenia ormood disorders in the follow-up. This study aimed todetermine sociodemographic and clinical features ofpatients diagnosed with acute and transient psychoticdisorder, as well as changes in the diagnosis in a 5 yearthe follow-up.Method: The sample was constituted by patients whodiagnosed with acute and transient psychotic disorderduring their hospitalization in 2010. Medical records ofthese patients were examined restrospectively in 2015,including past 5 years.Results: In a total number of 47 patients, 17 were femaleand 30 were male. Mean age was 32.2±9.9 years. Themajority were single (51.1%) unempoyed (85.1%) andliving with their families (87.2%). The rate of patientswho have been rehospitalized was 51.1%, and 8.5% ofpatients have not applied again. Most common diagnoses in the follow-up were atypical psychosis (38.3%),schizophrenia (25.5%) and bipolar affective disorder(12.8%). In 2 patients the diagnosis was psychotic disorder due to alcohol/substance abuse and 3 patients had aneurological disease.Conclusion: This study revealed that the diagnosis ofacute and transient psychotic disorder, have beenchanged to atypical psychosis or schizophrenia in the follow-up in the majority of patients. Therefore, the caseswho diagnosed with acute and transient psychotic disorder, especially if they carry risk factors, should be evaluated in detail in terms of prodromal period of schizophrenia. Furthermore, a close follow-up would be beneficial
