Açık kalp cerrahisinde postoperatif ekstübasyon sonrası bıpap ve cpap ile yapılan non-invazif ventilasyonun etkileri

Özet
Amaç: Açık kalp cerrahisi geçiren hastalarda postoperatif ekstübasyon sonrası İki Dereceli Pozitif Hava Yolu Basıncı (BIPAP) ve Sürekli Pozitif Hava Yolu Basıncı (CPAP) ile yapılan non-invazif ventilasyonun etkilerini değerlendirmektir. Yöntem: Tüm hastalara standart genel anestezi uygulandı. Operasyon sonrası yoğun bakım ünitesinde (YBÜ) ekstübe edilen hastalar randomize olarak üç gruba ayrıldı. Bir gruba ekstübasyon sonrası 30 dakika BIPAP, diğer gruba 30 dakika CPAP uygulandı. Devamında her iki gruptaki hastalara nazal kanül ile 5 L dk-1 oksijen verildi ve ekstübasyon sonrası 6. saatte kesildi. Kontrol grubunda ekstübasyonun ardından nazal kanül ile 5 L dk-1 oksijen verildi ve ekstübasyon sonrası 6. saatte kesildi. Her üç gruptaki hastaların kalp hızı, ortalama kan basıncı ve juguler venöz oksijen satürasyon değerleri ekstübasyondan 30 dakika önce ve ekstübasyondan 30 dakika sonra kaydedildi. PaO2, PaCO2, SaO2 ve pH verilerine, hasta YBÜ’ye alınır alınmaz, ekstübasyondan önce, ekstübasyondan 30, 120 ve 360 dakika sonra bakıldı. Bulgular: Demografik ve intraoperatif veriler, kalp hızı, ortalama kan basıncı, SaO2, juguler venöz oksijen satürasyonu ve pH değeri açısından gruplar arasında anlamlı bir fark saptanmadı, PaCO2 değerinde üç grup arasında anlamlı farklılık gözlenmezken, PaO2 değeri BIPAP ve CPAP grubunda kontrol grubuna göre anlamlı olarak yüksek bulundu. Sonuç: Açık kalp cerrahisi sonrası postoperatif dönemde ekstübasyon sonrası uygulanan BIPAP ve CPAP’ın arteriyel oksijenasyonu iyileştirdiği ancak pCO2’yi değiştirmediği kanısına varıldı.
Abstract
Objective: The objective of this study is to evaluate the effects of non-invasive BIPAP and CPAP ventilation after postoperative extubation on the patients went to open heart surgery. Method: Standard general anesthesia is performed to all of the patients. Patients are extubated in the intensive care unit (ICU) and they are randomly divided into three groups. After the extubation, BIPAP is performed for one the groups for 30 minutes and CPAP is performed for the other group for 30 minutes. After the non-invasive ventilations, 5 L min-1 O2 is applied with nasal canule to all of the patients and it is stopped after 6 hours from the extubation. In the control group, only 5 L min-1 O2 is applied with nasal canule after extubation and it is stopped 6 hours after the extubation. In all groups, heart rates, mean arterial pressures and central venous oxygen saturation values are noted before 30 minutes and after 30 minutes from the extubation. PaO2, PaCO2, SaO2 and pH values are noted immediately when the patient is taken to the ICU and 30 minutes, 120 minutes and 360 minutes after the extubation. Results: Demographic and intraoperative values, heart rates, mean arterial pressures, SaO2, central venous oxygen saturation and pH values are evaluated and there is no statistically significant difference is determined between the groups. While no significant difference in PaCO2 values between the groups is determined, PaO2 values are found significantly higher in the BIPAP and CPAP groups than the control group. Conclusion: After this study, we suggest that BIPAP and CPAP performed after extubation after open heart surgery get higher arterial oxygenation levels but do not effect arterial pCO2 values.
