Testis Rüptüründe Konservatif Yaklaşım: Nadir Bir Olgu ve Tedavisi

Özet

Testis rüptürü oldukça nadirdir ve cerrahi tedavi ile onarım klasik tedavi olarak tanımlanır. On üç yaşında erkek çocukta iki gün önce futbol oynarken top çarpması nedeniyle testiküler travma oluşmuştu. Sağ skrotumda ödem, şişlik, kızarıklık ve ağrı vardı. Skrotal doppler sonografide sağ testis anteriorunda süperior yerleşimli tunica albugineadan mediale protrude olan 18X15 mm boyutlarında testis parankimi izlendi (tunica albuginea yırtığı sonrası testis rüptürü). Geç getirilen olgu koservatif tedavi ile takibe alındı. Yatışının 4. günü kontrol skrotal doppler sonografide hematomun küçülmesi ve testis kan akımının normal olarak değerlendirilmesi üzerine hasta taburcu edildi. Erken tanımlamalarda cerrahi tedavi benimsenir. Gecikmiş olguda konservatif tedavi olumlu sonuç verdi. Solid organ yaralanmalarında son dönemlerdeki konservatif yaklaşımı bu olgularda da düşünebiliriz.

Abstract

Testicular rupture is quite rare and surgical treatment is defined as the classical treatment. Testicular trauma was occurred in a 13-year-old boy as a result of ball strike while playing football two days ago. There were oedema, swelling, rash and pain in the right scrotum. The scrotal doppler sonography revealed an 18x15 mm-size testis parenchyma protrusion located superior in the anterior part of the right testis to the medial of the tunica albuginea. Late delivered case was followed up with conservative treatment. On the 4th day of hospitalization, the patient was discharged since the hematoma revealed smaller in the control doppler US and testis blood flow was evaluated as normal. Surgical treatment is accepted as the therapy method by early diagnosed cases. By this patient, conservative treatment method also had positive results. We are in the opinion that the conservative approach by late diagnosed solid intraabdominal traumas may be considered also by such cases.
