Yoğun bakım ünitesinde takip edilen intoksikasyon olgularında plazmaferez etkinliğinin değerlendirilmesi

Özet
Amaç: Bu çalışmada, yoğun bakım ünitemizde (YBÜ) takip edilen intoksikasyon olgularında uygulanan plazmaferez tedavisinin etkinliğinin değerlendirilmesi amaçlanmıştır. Yöntem: Ocak 2014 - Aralık 2015 tarihleri arasında YBÜ’de takip edilen ve plazmaferez uygulanan intoksikasyon olgularının demografik özellikleri, plazmaferez öncesi ve sonrası AST, ALT, LDH, CK ve total bilirubin değerleri kaydedildi. Yapılan plazmaferez tedavisi öncesi ve sonrasına ait bu değerler karşılaştırılarak işlemin etkinliği değerlendirildi. Bulgular: Çalışmaya 38 hasta dahil edilmiş olup, medyan 5,5 (1-106) günlük yatış süresinin sonunda 32 (%84,2) hasta taburcu edilmiş, 6 (%15,8) hasta ise kaybedilmiştir. Bu hastalara toplamda 61 (minimum 1, maksimum 4) kez plazmaferez uygulandı. Plazmaferez öncesi ve sonrası AST, ALT, LDH, CK ve total bilirubin değerleri karşılaştırıldığında plazmaferez sonrası bu değerlerde istatistiksel anlamlı düşme belirlendi (sırasıyla; p değeri <0001, <0.001, <0,001, =0,014, <0,001). Medyan 5,5 günlük yatış süresi sonunda eksitus olan hastalarda plazmaferez öncesi AST ve ALT değerleri istatistiksel anlamlı olarak daha yüksek bulundu (sırasıyla p değeri = 0.014, 0,046). Sonuç: Yoğun bakım ünitesinde çoğunlukla intoksikasyon nedeni ile uyguladığımız plazmaferez tedavisi laboratuvar değerlerinde hızlı iyileşme sağlayan güvenli bir yöntemdir. Ancak geliş AST ve ALT değerleri çok yüksek olan hastalarda mortalite üzerine olumlu etkisi gösterilememiştir.
Abstract
Objective: In this study we aimed to evaluate the effectiveness of plasmapheresis in the intoxicated patients who were followed up in our ıntensive care unit (ICU). Method: Demographic characteristics, AST, ALT, LDH, CK and total bilirubin levels before and after plasmapheresis treatment of intoxicated patients in our ICU between January 2014 and December 2015 were recorded retrospectively. The effectiveness of plasmapheresis was evaluated by comparing the values before and after the treatment. Results: Thirty - eight patients were included in the study. At the end of a median 5.5 (1-106) days of ICU stay 32 (84.2%) patients were discharged, and 6 (15.8%) patients died. Plasmapheresis was applied to the patients for a total of 61 (minimum 1, maximum 4) times. When AST, ALT, LDH, CK and total bilirubin levels before and after plasmapheresis were measured, and significant decreases in these levels after plasmapheresis were determined (p<0.001, <0.001, <0.001, =0.014, <0.001, respectively). In the patients who died at the end of a median 5.5 days of ICU stay AST and ALT levels before plasmapheresis were found to be statistically significantly higher (p=0.014, 0.046, respectively). Conclusion: Plasmapheresis treatment mostly used for intoxications in ICU, is a safety method that provides quick improvement in biochemical values. But the positive effect of plasmapheresis on mortality could not be shown in the patients with very high levels of admission AST and ALT levels.
