Colchicine Intoxication in Children: Four Case Reports

Özet
Kolşisin, colchium autumnale bitkisinden elde edilen doğal bir alkaloiddir. Ailevi Akdeniz ateşi, amiloidozis, gut, Behçet hastalığı ve inflamatuar bağırsak hastalığının tedavisinde kullanılmaktadır. Bu yazıda, geçen yıl kliniğimize başvuran dört kolşisin intoksikasyonu olgusu sunuldu. İki hasta kolşisini kazara yutarken diğer ikisi intihar amaçlı almıştı. İlk hastanın klinik belirtileri gastroenterit, ateş ve alopesi idi. Bu hastada renal, hepatik disfonksiyon ve rebound lökositoz gelişti. Hastaya destek tedavisi verildi ve hasta taburcu edildi. İkinci hastanın fiziki muayenesi taşikardi dışında normaldi. İlacın alımından yirmi saat sonra hastada lökositoz gelişti. Hastaya plazmaferez ve lökoferez yapıldı. Alımdan 39 saat sonra hastada ani kardiyak arrest gelişti ve hasta öldü. Üçüncü hastanın klinik belirtileri gastroenterit, ateş ve alopesi idi. Hastada hepatik disfonksiyon ve rebound lökositoz gelişti. Destek tedavisi verildi ve hasta taburcu edildi. Son hastada klinik belirti ve laboratuvar bulgusu yoktu ve hasta taburcu edildi. Doktorlar ve diğer sağlık bakım çalışanları olası kolşisin intoksikasyonu semptomlarına karşı tetikte olmalıdır. Doz aşımı semptomlarının erken tespiti tedavinin başarısı için önemlidir.
Abstract
Colchicine is a natural alkaloid which is derived from the colchium autumnale plant. It is used in the treatment of familial Mediterranean fever, amyloidosis, gout, Behçets disease, and inflammatory bowel disease. In this article, we present four colchicine intoxication cases who were admitted to our clinic last year. While two patients ingested colchicine accidentally, other two took it for attempted suicide. First patients clinical signs were gastroenteritis, fever, and alopecia. This patient developed renal, hepatic dysfunction, and rebound leucocytosis. Patient was given support treatment and discharged. Second patients physical examination was normal except for tachycardia. Twenty hours after taking the medication, patient developed leucocytosis. The patient was performed plasmapheresis and leukapheresis. Thirty-nine hours after intake, patient developed sudden cardiac arrest and died. Third patients clinical signs were gastroenteritis, fever, and alopecia. Patient developed hepatic dysfunction and rebound leucocytosis. Support treatment was given and patient was discharged. Last patient had no clinical sign or laboratory finding and was discharged. Doctors and other healthcare employees should be alert for possible colchicine intoxication symptoms. Early detection of overdose symptoms is significant in terms of treatment success.
