Sulkus Katlanabilir Göz İçi Lens İmplantasyonu ile Sulcus Polimetilmetakrilat Göz İçi Lens İmplantasyonu Sonrası Göz İçi Basıncı Değişimlerinin Karşılaştırılması

Özet
Amaç: Fakoemülsifikasyon esnasında arka kapsül perforasyonu nedeni ile katlanabilir çok parçalı hidrofobik akrilik göz içi lensi (GİL) sulkusa implante edilen olgular ile sulkusa implante edilen polimetilmetakrilat (PMMA) GİL implantasyonlu olguların postoperatif göz içi basıncı (GİB) değişimlerini karşılaştırmak.Gereç ve Yöntem: Fakoemülsifikasyon uygulanan, arka kapsül perforasyonu nedeni ile sulkusa katlanabilir çok parçalı hidrofobik akrilik GİL implante edilen 30 hastanın 30 gözü çalışma kapsamına alındı (Grup1). Fakoemülsifikasyon uygulanan , arka kapsül perforasyonu nedeni ile sulkusa PMMA GİL implante edilen 31 hastanın 31 gözü çalışma kapsamına alındı (Grup 2). Grup 1 ve Grup 2'de gereken olgularda ön vitrektomi yapıldıktan sonra 30 göze katlanabilir çok parçalı hidrofobik akrilik GİL, 31 göze PMMA GİL sulkusa yerleştirildi. Preoperatif, postoperatif 24. saat, 7. gün, 1. ay, 3. ay ve 6. ayda Goldmann aplanasyon tonometrisi ile ölçülen GİB değerleri iki grup arasında karşılaştırıldı.Bulgular: Preoperatif ortalama GİB değerleri, Grup 1 17.7±1.7 mmHg, Grup-2 17.8±0.8 mmHg idi. İki grup arasında preoperatif GİB değerleri açısından anlamlı bir fark yoktu (p>0.05). Grup 1 ve Grup 2'de postoperatif ölçülen GİB değerleri ile preoperatif GİB değerleri arasında istatistiksel olarak anlamlı farklılık yoktu.(p>0.05). İki grup arasında da izlem boyunca istatistiksel olarak anlamlı GİB farklılığı yoktu (p>0.05).Tartışma: Fakoemülsifikasyon cerrahisinde sulkusa GİL implante etmek zorunda kalınan olgularda erken postoperatif dönemde göz içi basıncında önemli bir değişiklik olmamaktadır.
Abstract
Purpose: To compare the postoperative intraocular pressure (IOP) changes in cases with sulcus fixtated foldable multipiece hydrophobic acrylic intraocular lens (IOL) implantation because of the posterior capsule perforation during phacoemulsification and with sulcus fixated polymethylmethacrylate (PMMA) IOL implantation.Material and Methods: 30 eyes of 30 patients with sulcus fixated foldable multipiece hydrophobic acrylic IOL because of the posterior capsule perforation during phacoemulsification were included in this study (Group 1). 31 eyes of 31 patients with sulcus fixated PMMA IOL because of the posterior capsüle perforation during phacoemulsification of the posterior capsule perforation during phacoemulsification (Group 2) After performing anterior vitrectomy to necessary cases, in 30 eyes, a foldable multipiece hydrophobic acrylic IOL and in 30 eyes, a PMMA IOL was implanted in the sulcus in Group 1 and Group 2. The IOP values which has been measured by Golmann applanation tonometry preoperatively, 24th hour, 7th day, 1st month, 3th month and 6th month posptoperatively were compared among two groups.Results: The preoperative mean IOP values were 17.7±1.7 mmHg in Group 1 ands 17.8±0.8 mmHg in Group 2. There was no statistical significant difference between the mean IOP values of two groups preoperatively (p>0.05). There was no statistical significant difference between the postoperative measured IOP values in Group 1 and 2 (p>0.05). There were no statiscial significant differences in postoperative IOP values between two groups (p>0.05).Conclusion: In cases which the IOL has to be implanted to the sulcus during phacoemulsifacation surgery, there is no significant change in IOP in early postoperative period.
