?izofreni Hastalarynda Hastalyk Süresinin Klinik Özelliklere Etkisi

Özet
Amaç: ?izofreni nüfusun %1'ini etkilemekte olup klinikseyri oldukça de?i?kendir. Bu çaly?mada ?izofreninin uzamy? hastalyk sürecinde klinik görünümünün tanymlanmasyna katkyda bulunmak amacyyla 10 yyldan kysa sürelive 15 yyldan uzun süreli ?izofreni hastalarynyn klinik özellikler açysyndan kar?yla?tyrylmasy planlanmy?tyr. Yöntem:Çaly?maya ?izofreni tanysy konmu? 10 yyldan kysa süretakip edilen 71 hasta ve 15 yyldan uzun süre takip edilen53 hasta dahil edilmi?tir. Tüm hastalara Pozitif ve NegatifSendrom Ölçe?i (PNSÖ), Calgary ?izofrenide DepresyonÖlçe?i (C?DÖ), Ya?amdan Ho?nutluk ve Doyum AnketiKysa Formu (Quality of Life Enjoyment and SatisfactionQuestionnarie; Q-LES-Q), Ekstrapiramidal BelirtileriDe?erlendirme Ölçe?i (EBDÖ), UKU (Ugvalgfor KliniskeUndersgelser) Yan Etki Derecelendirme Ölçe?i, YçgörününÜç Bile?enini De?erlendirme Ölçe?i (YÜBDÖ) ve MoriskyTedavi Uyum Ölçe?i (MTUÖ) uygulanmy?tyr. Bulgular:PNSÖ Negatif belirtiler alt ölçe?i, Ç?DÖ, YÜBDÖ ve MTUÖpuanlary 15 yyl ve daha uzun hastalyk süresine sahip hastagrubunda 10 yyl ve daha kysa hastalyk süresine sahiphasta grubuna göre istatistiksel olarak anlamly ?ekildeyüksektir. UKU yan etki de?erlendirme ölçe?inin nörolojik alt ölçek puanlary iki grup arasynda kar?yla?tyryldy?ynda;15 yyl ve daha uzun hastalyk süresine sahip hasta grubunda istatistiksel olarak anlamly ?ekilde daha yüksek bulunmu?tur. Sonuç: Bu çaly?ma sonucunda, ?izofreni tanylyhastalarda hastalyk süresi uzadykça negatif ve depresifbelirtilerde artmanyn yany syra hastalaryn hastaly?a dairiçgörü düzeylerinin ve ilaç uyumlarynyn artty?y gösterilmi?tir. Uzamy? hastalyk süresine sahip ?izofreni hastalarynda; ?izofreni hastaly?ynyn gidi? özelliklerinin neleroldu?u ve bu gibi durumlarda hasta takibinde dikkatealynacak unsurlara yönelik daha büyük ölçekli, ileriyedönük planlanmy? çaly?malara ihtiyaç oldu?u görülmektedir
Abstract
Object: Schizophrenia affects 1% of the population andthe clinical course is highly variable. In this study, weaimed to compare the clinical characteristics of patientswith schizophrenia longer than 10 years and longer than15 years in order to contribute to the identification ofthe clinical appearance of schizophrenia in the prolonged disease process. Methods: 71 patients who werediagnosed with schizophrenia for a short period of 10years and 53 patients who were followed for a period of15 years were included. All of the patients were assessedby the Positive and Negative Syndrome Scale (PANSS),the Calgary Schizophrenia Depression Scale (CSDS), theQuality of Life Enjoyment and Satisfaction Questionnaire(Q-LES-Q), the Extrapyramidal Symptom Rating Scale TheUKU (UgvalgforKliniskeUndersgelser) Side Effect RatingScale, the Schedule for Assessing the Three Componentsof Insight and the Morisky Treatment Compliance Scale(MCQS) were administered. Results: The scores of PANSSNegative symptoms subscale, CBSS, BPRS and MTUSscores were statistically significantly higher in the patientgroup with a disease duration of 15 years or more thanin the group with a disease duration of 10 years or less.When the neurological subscale scores of the UKU sideeffect assessment scale were compared between the twogroups; they were statistically significantly higher in thepatient group with a disease duration of 15 years orlonger. Conclusion: As a result of this study, it has beenshown that, in patients with schizophrenia, the longerthe illness is, the more negative and depressive symptoms increase, as well as the insight into the patient's illness and the increased drug compliance. It is seen thatthere is a need for larger scale, prospective planned studies for schizophrenic patients with prolonged diseaseduration, to understand what is going on schizophreniaand for factors to be taken into account in such cases.
