Miyeloproliferatif neoplazmlarda JAK2V617F mutasyonu ile trombosit fonksiyonları arasındaki ilişkinin değerlendirilmesi

Özet

Amaç: Miyeloproliferatif neoplazm (MPN), hemostaz ve tromboz anomalileri ve akut lösemiye ilerleyebilen klonal bir durumdur. PV (polisitemi vera), ET (esansiyel trombositoz) ve IMF (idiyopatik myelofibrozis) bcr- abl negatif miyeloproliferatif neoplazm (MPN)’dır. WHO (Dünya Sağlık Örgütü)’yı göre PV, ET ve İMF’nin tanısında JAK2V617F mutasyonu varlığı tanı kriterlerinden biridir. Bu çalışmada, 60 MPN tanılı hastada JAK2V617F mutasyonu ile trombosit fonksiyonları arasındaki ilişki araştırılması amaçlandı. Yöntem: Çalışma, Pamukkale Üniversitesi Hematoloji Polikliniğine başvuran ve yapılan incelemelerde WHO’nun revize edilen kriterlerine göre MPN tanısı alan hastalarda yapılmıştır. Çalışma 30’u PV, 28’i ET ve 2’si IMF olmak üzere toplam 60 hastayı içermektedir. Hastaların ADP, ristosetin, epinefrin ve kollagen ile trombosit agregasyon testleri yapıldı. Bulgular: Çalışmaya katılan 60 hastanın 30’unda PV, 28’inde ET ve 2’sinde IMF vardı. Yirmi sekiz hasta kadın (%46,6) ve 32 erkek (%53,3) idi. JAK2 mutasyonu 39 hastada (%65) saptanmıştır. JAK2 mutasyonunun sıklığı PV için %83,3, ET için %42,9 ve IMF için %100 olarak belirlendi. Trombosit fonksiyon bozukluğu hastaların %80’inde bulundu. Bu oran, JAK2 mutasyonu pozitif olan hastalarda %76,9 ve JAK2 mutasyonu negatif olan hastalarda %85,7 idi. JAK2V617F mutasyonu pozitif hastalar ile JAK2V617F mutasyonu negatif olan hastalar arasında epinefrin, ristosetin, kollajen ve ADP değerleri bakımından istatistiksel olarak anlamlı bir ilişki bulunamamıştır. Sonuç: Çalışmamızda, JAK2 mutasyonu ile trombosit fonksiyonları arasında ilişki bulunamadı. Sonuç olarak, MPN’ larda görülen trombosit disfonksiyonu JAK2 mutasyonundan bağımsızdır.

Abstract

Objective: Myeloproliferative neoplasm (MPN) is a clonal condition that may progress to hemostasis and thrombotic anomalies and acute leukemia. Polycythemia vera (PV), essential thrombocytosis (ET) and idiopathic myelofibrosis (IMF) are among bcr-abl negative myeloproliferative neoplasms (MPNs). According to WHO (World Health Organization), presence of the JAK2V617F mutation is one of the diagnostic criteria of PV, ET and IMF. In this study, it was aimed to investigate the relationship between JAK2V617F mutation and platelet function in a patient with the diagnosis of 60 MPN. Method: The study was conducted in Pamukkale University hematology polyclinic patients and in patients who were diagnosed with MPN according to WHO revised criteria. The study population had 60 participants including 30 PV, 28 ET and 2 IMF patients. Platelet aggregation tests were performed for ADP, ristocetin, epinephrine and collagen, using an aggregometer device. Results: Among the 60 patients participating in the study, 30 had PV, 28 had ET and 2 had IMF. Twenty-eight patients were women (46.6%) and 32 were men (53.3%). The presence of JAK2 mutation was detected in 39 patients (65%). The frequency of the JAK2 mutation was detected to be 83.3% for PV, 42.9% for ET, and 100% for IMF. Platelet function disorder was found in 80% of the patients. While it was detected in 76.9% and 85.7% of patients with positive, and negative JAK2 mutation, respectively. No statistically significant relationship was found in terms of epinephrine, ristocetin, collagen and ADP values between patients with positive, and negative JAK2V617F mutations. Conclusion: In our study, no correlation was found between JAK2 mutation and platelet function. In conclusion, the platelet dysfunction seen in MPNs is independent of the JAK2 mutation.
