ZOR ENTÜBASYON KRİTERLERİNİN DEĞERLENDİRİLMESİ: PROSPEKTİF GÖZLEMSEL ÇALIŞMA

Özet

Giriş: Anesteziye bağlı komplikasyonları azaltmak için zor hava yolu anestezi öncesi dikkatli bir değerlendirme ile belirlenmeli ve gerekli önlemler alınmalıdır. Bu çalışmada preoperatif değerlendirme testlerinin güvenilirliği araştırılmıştır. Gereç ve Yöntem: Bu prospektif gözlemsel çalışma 1 yıllık sürede genel anestezi altında entübe edilecek 18 yaş üstü ASA I-III risk grubunda elektif cerrahi yapılacak hastalar ile yapıldı. Mallampati (III ve IV riskli), tiromental mesafe (≤6,5 cm riskli), sternomental mesafe (≤12,5 cm riskli), atlantooksipital eklem hareketliliği (≤35 0 riskli), ağız açıklığı (ön kesici dişler arası mesafe) (≤3,5 cm altı riskli), vücut kitle indeksi (>30 riskli), üst dudak ısırma testi (ÜDIT) (SINIF III riskli) ölçümleri yapıldı. Bu zor entübasyon belirleme yöntemlerinin Cormack-Lehane sınıflandırmasına (CL) (III ve IV zor entübasyon kabul edildi) ve uygulayıcı bağımlı zor entübasyon(deneyimli bir uygulayıcının 3 ve daha fazla denemede entübe edememesi) belirlemedeki başarısı tek tek ve birlikte değerlendirildi. Bulgular: 622 hastadan zor entübasyon tanımına uyan 23 (%3,7) hasta ve CL skoru III ve IV olan 61 (%9,8) hasta oldu. Tek başına değerlendiğinde mallampati testinin spesivitesi % 65,2 sensitivesi %89,2 ÜDIT’nin spesivitesi %43,5 sensitivitesi %97,2 olarak bulundu. En iyi sonuç mallampati, ÜDIT ve VKİ’nin beraber değerlendirilmesi ile elde edildi (spesivitesi %95,7 sensitivitesi %75,1 olarak bulundu). Sonuç: Çalışmamız göstermiştir ki kullandığımız preopertif değerlendirme testleri ve kombinasyonları ile zor entübasyon gelişebilecek hastaların büyük çoğunluğu öngörülebilse de, tamamını önceden tahmin edebilmek mümkün değildir.

Abstract

Introduction:Difficult air way should be determined by careful evaluation before anesthesia and necessary preventions should be taken in order to reduce complications due to anesthesia. In this study, the reliability of preoperative evaluation tests was investigated. Matherials and Methods: This prospective observational study was performed among patients who underwent elective surgery under general anesthesia, ASA I-III risk group and over 18 years old, for a period of one year. Mallampati (III and IV risk), tyhromental distance (≤ 6,5 cm risk), sternomental distance (≤ 12,5 cm risk), atlantooccipital joint mobility (≤ 35° risk), mouth openness (≤ 3,5 cm), body mass index (> 30 risk) were measured and upperlip bite test (ULBT Class III risk) was applied. These difficult intubation detection methods were evaluated alone and together in order to determine the success of Cormack-Lahen classification (CL) (III and IV were considered as difficult intubation) and practitioner-dependent difficult intubation (3 or more attempts of an expierenced practitioner). Results: 622 patients were included for the study. 23 patients (3,7 %) compliant with the definition of difficult intubation and 61 patients (9,8 %) with CL scores III and IV. When assesment was held alone, thes pecificity of Mallampati was 65,2 %, sensitivity was89,2 % and the specificity of ULBT was 43,5 %, sensitivity was 97,2 %. The best results were obtained by evaluating Mallampati, ULBT and BMI together (specificity 95,7 %, sensitivity 75,1 %). Conclusion: Our study has shown that although most of patients with difficult intubation can be determined by preoperative evaluation tests alone and together, it is not possible to predict the complete.
