Özet

Giriş: Safra kanalı içine açılan karaciğer kist hidatik; olgularında morbidite ve mortaliteye yol açmaktadır. Çalışmamızda karaciğer kist hidatikli olgularda kist içine açılan safra kanalı ile ilişkili faktörleri araştırmayı amaçladık. Gereç ve Yöntem: Karaciğer kist hidatiği nedeniyle opere edilen olgular ameliyat anında kist boşluğunda safra görülenler (kistobilier fisül) (Grup I) ve safra görülmeyenler (kistobilier fistül görülmeyenler) (Grup II) şeklinde iki gruba ayrıldı. Gruplar yaş, cinsiyet, kist boyutu ve karaciğer enzim değerleri vb. faktörler açısından karşılaştırıldı. Bulgular: Çalışmamıza 30(%68)’i kadın ve 14(%34)’ü erkek olmak üzere toplamda 44 olgu dahil edildi. Grup I’de 7(%16) ve Grup II’de 37(%84) olgu mevcuttu. Kist boyutunun Grup I ve Grup II ortanca değerleri sırasıyla 100(60-130) ve 80(30-180) cm idi (p>0,05). Olguların gruplara göre kist boyutu ve diğer kategorik değişkenleri incelendiğinde gruplar arasında istatistiksel olarak anlamlı fark bulunmadı (p>0,05). Olguların gruplara göre sürekli değişkenleri incelendiğinde ise Grup I olgularının INR değerleri Grup II olgularının INR değerlerinden anlamlı yüksek idi (p<0,05). Sonuç: Kistobilier fistül varlığının ameliyat öncesi tanınması ile ilgili spesifik faktörler olmadığını gördük. En iyi tanı yönteminin ameliyat anında kist poşunun ayrıntılı eksplorasyonu olduğunu düşünmekteyiz.

Abstract

Introduction: Liver cyst hydatid opened in bile duct causes to morbidity and mortality in patients. We aimed to investigate the factors associated with bile duct opening into the cyst in cases with hydatid cycts of liver. Material and method: Patients who underwent surgery due to liver cyst hydatid were divided into 2 groups: Bile seen in the cyst cavity at the time of surgery (cystobiliary fistula) (Group I) and no bile seen (no cystobiliary fistula) (Group II). The groups were compared in terms of age, gender, cyst size and liver enzyme values etc. Results: 44 cases of 30 (68%) female and 14(34% ) male were included in the study. There were 7 (16%) cases in Group I and 37 (84%) cases in Group II. Group I and Group II median values of the cyst size were 100 (60-130) and 80 (30-180) cm, respectively (p> 0.05). The cyst size and other categorical variables were compared according to the groups, there was no statistically significant difference between the groups (p> 0,05). The continuous variables of the cases were examined according to the groups, the INR values of Group I cases were significantly higher than the INR values of Group II cases (p<0,05). Conclusion: We concluded that there are no specific factors related to the preoperative diagnosis of the presence of cystobiliary fistula. We think that the best diagnostic method is a detailed exploration of the cyst pouch at the time of surgery.
