Özet
Kök hücre nakli gerek, malign gerekse benign birçok hastalıkta küratif bir tedavi seçeneğidir. Günümüzde otolog kök hücre nakillerin %99’u, allojenik kök hücre nakillerin ise %70’inden fazlasında kök hücre kaynağı olarak periferik kök hücre kullanılmaktadır. Periferik kan kök hücre miktarı kemik iliğine göre daha azdır. Bu nedenle kemik iliğinden periferik kana kök hücre geçişi sağlamak gerekir. Bu işlem mobilizasyon işlemi ve kullanılan ilaç mobilizasyon ajanı olarak adlandırılır. İlikten periferik kana hematopoietik kök hücrelerin mobilizasyonu için sıklıkla G-CSF ya tek başına ya da kemoterapi ile birlikte kullanılır. Allojenik periferik kök hücre mobilizasyonunda ise G-CSF kullanılmaktadır. Mobilizasyon başarısızlığında ise plerixafor GCSF kombinasyonu önemli bir tedavi seçeneğidir. Otolog ve allojenik olgularda optimal toplama zamanı periferik kandaki CD34+ hücre sayısı değerlendirilerek kararlaştırılır. Bunun için eşik değer >10-20/μL CD34+ hücre sayısıdır. Toplanan ürünün yeterliliği ise üründeki CD34+ hücre sayısıyla değerlendirilir. Deneyimli bir aferez ekibi için uygulanacak yöntemler, bu yöntemlere bağlı görülebilecek yan etkilerin iyi bilinmesi ve bu konuda hasta ve vericilerin bilgilendirilmesi yapılacak işlemin kalitesi açısından büyük önem taşımaktadır.
Abstract
Stem cell transplant is a curative treatment option for many malignant, and benign diseases. Today, peripheral stem cell is used as a source of stem cells for 99% of autologous stem cell transplants, and more than 70% of allogenic stem cell transplants as well. Hence the amount of peripheral blood stem cells is less than it is found in bone marrow. Therefore it is a necessity to ensure passage of stem cells from bone marrow into peripheral blood. This procedure is called as mobilisation and the drug used for this procedure is also called as a mobilisation agent. In general, G-CSF is used either alone or with chemotherapy for the mobilisation of hematopoietic stem cells from bone marrow into peripheral blood. However G-CSF is used for the mobilisation of allogenic peripheral stem cell. The combination of GCSF and plerixafor is a significant treatment option in the event of mobilisation failure. Optimal collection time for autologous and allogenic cases is determined based upon the assessment of CD34+ cell counts in peripheral blood. For this procedure the threshold of CD34+ cell counts is >10-20/μL The sufficiency of collected product is assessed by the number of CD34+ cells in the product. It is of great importance for an experienced apheresis team to be very knowledgeable with the methods applied and their side effects and informing patients and donors with regard to the quality of performed procedure.
