Özet

Motor nöron hastalığı ilerleyici kas atrofisi, refleks canlılığı, spastisite, fasikülasyon, yutma ve çiğneme güçlüğü, dizartri ile seyreden bir hastalıktır. Solunum yetmezliği başlıca ölüm nedenlerinden biridir. Polinöropati, periferik sinirlerin yaygın bir hastalığı olup kol ve bacaklarda ilerleyen kuvvetsizlik, bozulmuş duyu fonksiyonu ile ortaya çıkan bir hastalıktır. Bu olgu sunumumuzda transüretral prostat rezeksiyon planlanan, motor nöron ve polinöropati hastalıklarının birlikte olduğu bir hastada nöromüsküler bloker kullanmadan uyguladığımız genel anestezi yaklaşımımız aktarılmaktadır. Preoperatif değerlendirmede kuadriparezi, alt ekstremitede hipoestezi, dizartri ve fasikülasyonları bulunan hastanın spontan solunum eforu yeterliydi. Mevcut nöromüsküler patolojilerle etkileşme ve motor blokta uzama riski nedeniyle rejyonel anestezi uygulanmayan hastada inhalasyon tekniği ile genel anestezi planlanmıştır. Yine genel anestezi sırasında uzamış kas paralizisi ve solunum yetmezliği nedeniyle nöromüsküler bloker uygulanmamıştır. Propofol (1 mg kg'1) ile indüksiyon yapılan hastada anestezi idamesinde %50 N20/%50 02 karışımı içerisende %l-2 sevofluran uygulanmış ve solunum desteği laringeal maske ile sağlanmıştır. Opioidler anestezi planında yer almamıştır. Ameliyat süresince herhangi bir problem izlenmeyen; postoperatif spontan solunum eforu yeterli, hemodinamik ve nörolojik açıdan stabil olan hasta servise gönderilmiştir. Sonuç olarak ilerleyici kas güçsüzlüğüyle seyreden nörolojik hastalıklarda nöromüsküler bloker ajan kullanılmadan volatil anesteziklerle genel anestezi uygulamasını başarılı postoperatif erken derlenme ile beraber solunum yetmezliği, motor kayıp gibi komplikasyonların gelişmemesi açısından seçilecek anestezi yöntemi olabileceğini düşünmekteyiz.

Abstract

Motor neuron disease is a disorder which has symptoms such as progressive muscle atrophy, exaggerated reflexes, spasticity, fasciculation, difficulty in chewing and disartry. The main cause of mortality is respiratory failure. Polyneuropathy is a common disorder of peripheral nerves which also manifests with progressive weakness in extremities, impaired sensorial function. In this case report we present general anesthesia approach of a patient with motor neuron disease accompanied with polyneuropathy scheduled for transurethral prostate resection without using neuromuscular blockers. The respiratory effort of the patient with quadriparesis, hipoesthesia in lower extremities, disartry and fasciculation was sufficient. General anesthesia with inhalational techniques was planned because of interaction .with available neuromuscular pathologies and the risk of prolonged motor blockade. Also neuromuscular blockers weren't administered owing to prolonged muscle paralysis and respiratory failure during general anesthesia procedure. Anesthesia was induced with 1 mg kg4 propofol, and % 1-2 sevoflurane concentrations in 50% N20/50% 02 were adjusted to maintain anesthesia, respiratory support was provided with a laryngeal mask airway and opioids weren't used. The patient who has no problem during the operation and has enough respiratory effort, stable in hemodyanic parameters and neurologic finding was transpered to the ward. In conclusion we suggest that general anesthesia with volatile anesthetics without neuromuscular blocking is a preferable anesthesia technique for satisfactory postoperative rapid recovery and prevention of complications such as respiratory failure, motor dysfunction in progressive neurologic disorders with muscle weakness.
