ORTA-İLERİ DÜZEY DİYABETİK AYAK ENFEKSİYONLARINDA MİKROBİYOLOJİK SPEKTRUM VE ANTİBİYOTİK DİRENÇ ORANLARI

Özet
Giriş: Orta ve ciddi enfekte diyabetik yaraları bulunan hastalarda polimikrobiyal, anaerob ve gram negatif etkenlerin sıklığı yüksektir. Bu hastalarda aynı zamanda antimikrobiyal direnç oranları da daha yüksek olduğu için tedavide sorunlu hasta grubunu oluşturmaktadırlar. Çalışmamızda bu hasta grubundaki bakteriyel etkenlerin dağılımını ve antibiyotik duyarlılık oranlarını araştırmayı amaçladık. Gereç ve Yöntem: 1 Haziran 2017 ve 31 Aralık 2017 tarihleri arasında müracaat eden komplike diyabetik ayak enfeksiyonu olan hastalar bu retrospektif tanımlayıcı çalışmaya alındı. Hastaların yara kültürlerindeki mikrobiyolojik etkenler ve bu etkenlerdeki antibiyotik duyarlılık profilleri hastane otomasyon sistemi üzerinden çıkarıldı. Bulgular: Çalışmaya alınan 46 hastanın 39’unda (%84.8) en az bir üreme olmuştu ve toplamda 62 mikrobiyolojik etkene ait verilere ulaşıldı. Üremelerin büyük çoğunluğunu gram negatif bakteriler(%75.8, n= 47) oluşturmaktaydı. Ayrıca 14 hastada (%22.58) gram pozitif bakteri ve bir hastada kandida üremesi mevcuttu. En sık gözlenen bakteriler; gram negatif etkenlerde Escherichia coli (n= 10, %16), Acinetobacter spp. (n= 7, %11.3), Morganella morganii (n= 7, %11.3), Pseudomonas spp. (n= 6, %9.68), Enterobacter cloacae (n= 5,%8.06) ve gram pozitif etkenlerde ise Enterococcus spp. (n= 5,%8.06) idi. Gram pozitif etkenlerin hiçbirinde vankomisin, teikoplanin, linezolid, daptomisin ve tigesikline karşı direnç saptanmadı. Diğer antibiyotiklere duyarlılık oranları düşüktü. Gram negatif etkenlerdeki antibiyotik duyarlılık durumu incelendiğinde, E. coli için amikasin ve meropenem duyarlılığının %100, imipenem, tigesiklin ve kolistin duyarlılığın %90 civarında olduğu görüldü. Pseudomonas spp., Acinetobacter spp., Enterobacter cloacae izolatlarında kolistin duyarlılığı %100’ dü. Gram negatif etkenlerin genel olarak en yüksek duyarlılığa sahip olduğu antibiyotik amikasin idi. Karbapenemler dâhil bunların dışında kalan tüm antibiyotiklere karşı duyarlılık oranları nispeten düşük düzeydeydi. Sonuç: Orta ve ciddi diyabetik ayak enfeksiyonlu olgularda gram negatifler etkenler daha sık görülmektedir. Bu olgularda gram pozitif bakterilerden enterokokların, gram negatif bakterilerden ise E. Coli türlerinin görülme sıklığında artış mevcuttur. Ek olarak bu hasta grubunda, özellikle de gram negatif etkenlerde antibiyotik direnç oranlarındaki yükseklik dikkat çekicidir.
Abstract
Introduction:The prevalence of polymicrobial, anaerobic and gram negative agents is high in patients with moderate and severe diabetic foot infection. These patients also have higher rates of antimicrobial resistance and therefore constitute a problematic patient group. In this study, we aimed to investigate the distribution of bacterial agents and the antibiotic resistance rates in this group. Material and Method: Patients with moderate-to-severe diabetic foot infection who were referred to our hospital between June 1, 2017 and December 31, 2017 were included in this retrospective descriptive study. Microbiological features in the wound cultures and antibiotic susceptibility profiles of these microbial agents were obtained using the hospital automation system. Results: Of the 46 patients included in the study, 39 (84.8%) showed at least one bacterial growth and a total of 62 wound cultures were identified. Gram negative bacteria were the majority with a rate of 75.8% (n = 47). In addition, 14 patients (22.58%) had gram positive culture identification. The most common bacterial agents in gram negative bacteria were Escherichia coli (n = 10, 16%), Acinetobacter spp. (n = 7, 11.3%), Morganella morganii (n = 7, 11.3%), Pseudomonas spp. (n = 6, 9.68%) and Enterobacter cloacae (n = 5, 8.06%), while, Enterococcus spp. (n = 5, 8.06%) was the most common in gram positive bacteria. No resistance to vancomycin, teicoplanin, linezolid, daptomycin and tigecycline was detected in any of the gram positive bacteria, while the antimicrobial susceptibility to other antibiotics was low. Among gram negative bacteria, E. coli had 100% sensitivity rate for amikacin and meropenem and 90% for imipenem, tigecycline and colistin. The susceptibility rate of colistin for Pseudomonas spp., Acinetobacter spp., Enterobacter cloacae was 100%. Amikacin showed the highest susceptibility rate against gram negative bacteria. However,susceptibility rates to all other antibiotics, including carbapenems, were relatively low in gram negative bacteria. Conclusion: Gram negative bacteria manifested high level resistance to most classes of antibiotics in patients with moderate-to-severe diabetic foot infections.There is an increase in the frequency of enterococci among gram positive bacteria and E. coli species among gram negative bacteria.Besides that, the high rate of antibiotic resistance in this group of patients, especially in gram negative bacteria, is noteworthy.
