İLERİ OTOSKLEROZ SONRASI KOKLEAR İMPLANT PROGRAMLAMA KRİTERLERİ

Özet
Amaç: Bu çalışmanın amacı, ilerlemiş otoskleroz sonrası koklear implantasyon uygulanmış bireylerde programlama parametrelerinin incelenmesidir. Yöntem: Yapılan bu retrospektif çalışmada, otoskleroza bağlı ileri/çok ileri derecede işitme kaybı sonrası en az bir yıldır koklear implant kullanan 11 birey çalışma grubuna; idiyopatik sebeplere bağlı ileri/çok ileri derecede işitme kaybı sonrası en az bir yıldır koklear implant kullanan 11 birey ise kontrol grubuna dahil edilmiştir. Çalışmada marka farkı gözetmeksizin, bir yıllık koklear implant kullanımı sonunda intrakoklear elektrotların impedans değişiklikleri, uyaran genişliği/süresi (pulse width /duration) ve uyaran hızları (rate) kontrol grubu ile karşılaştırılmıştır. İstatistiksel analizlerde non-parametrik testler kullanılmıştır. Bulgular: Çalışma grubunda yaş ortalaması 47,2 iken kontrol grubunda yaş ortalaması 46,18'dir. Tüm elektrotların impedans değerlerinin istatistiksel olarak farklı olduğu görülmüştür (p=.045; p .05). Çalışma ve kontrol gruplarının en rahat duyma ve eşik seviyesi çalışma grubunda kontrol grubundan daha yüksek elde edilmiş olmasına rağmen istatistiksel olarak anlamlı fark tespit edilmemiştir. Tartışma ve Sonuç: Otoskleroz etiyolojisine sahip bireylerde, otik kapsülde meydana gelen değişikliklere bağlı olarak impedans değişiklikleri olduğu ve özellikle bu değişimin bazal bölgedeki elektrotlarda olduğu görülmüştür. İmpedanslarda meydana gelen değişikliklerin uyaran hızı ve uyaran süresini etkilediği bilinmekle birlikte istatistiksel olarak anlamlı farklılık görülmemiştir. Anlamlı farklılık olmamasının çalışmada yer alan vaka sayısının az olması ile ilgili olduğu düşünülmüştür. Vaka sayısının az olması çalışmanın sınırlığı olarak değerlendirilmiştir.
Abstract
Objective: The aim of this study was to evaluate programming parameters of cochlear implant users after advanced otosclerosis. Method: In this retrospective study, 11 individuals after severe/profound hearing loss due to advanced otosclerosis were included in the study group; 11 individuals severe or profound hearing loss due to idiopathic reasons were included in the control group. All of the participants use cochlear implants at least one year. Cochlear implant programming parameters at the end of the one-year usage, impedance changes, pulse width/duration and stimulus rates were compared. Non-parametric tests were used for statistical analysis. Results: The mean age was 47.2 in the study and 46.18 in the control group. The impedances on all electrodes were found to be statistically different (p=.045;p .05). Although the most comfortable loudness and threshold levels of the study group were higher than the control group, this difference was not statistically significant. Discussion and Conclusion: It has been observed that individuals with otosclerosis have impedance variations due to otic capsule changes and these changes are obvious on the basal part. It is known that changes in impedances could affect the stimulus rate and duration, however in current study there was no statistically significant difference. The lack of significant difference could be related to the small number of cases involved in the study. The inadequate number of cases could be accepted as limitation of the study.
