Obezite tanılı çocuk ve ergenlerde psikopatoloji, yaşam kalitesi ve ebeveyn tutumlarının değerlendirilmesi

Özet
Amaç: Bu çalışmada, obezite tanılı çocuk ve ergenlerdeki psikopatolojiyi, yaşam kalitesi algılarını, benlik saygılarını araştırmak, ayrıca ebeveyn tutumlarını ve başa çıkma stratejilerini ve bunların birbiriyle olan ilişkilerini araştırmak amaçlanmıştır. Yöntem: Çalışmamız AÜTF Hastanesi Çocuk Sağlığı ve Hastalıları Anabilim Dalı Çocuk Endokrinoloji Bölümüne başvuran, 8-18 yaşında VKİ standart sapması ikinin üzerinde olan, zihinsel gelişim geriliği ve psikiyatrik başvurusu olmayan 30 hasta ile yürütüldü. Kontrol grubuna 8-18 yaşlar arasında herhangi bir tıbbi hastalık öyküsü olmayan ve daha önce herhangi bir nedenle çocuk psikiyatrisi polikliniğine başvurmayan, hasta grubuyla yaş ve cinsiyet açısından eşleştirilmiş, gönüllü 30 çocuk ve ergen alındı. Çalışmaya katılan tüm çocuk ve ergenlere Okul Çağı Çocukları için Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi-Şimdi ve Yaşam Boyu Versiyonu uygulandı. Benlik saygısını değerlendirmek için ise Rosenberg Benlik Saygısı Ölçeği kullanıldı. Çocuk ve ergenlerin yaşam kalitelerini değerlendirmeye yönelik, çocuk ve ergenlere ve ebeveynlerine Çocuklar İçin Yasam Kalitesi Ölçeği verildi. Ebeveynlerin çocuk yetiştirme tutumlarının ve stresle başa çıkma becerilerinin değerlendirilmesi için anne-babalarına Aile Hayatı ve Çocuk Yetiştirme Tutumu Ölçeği ve Başa Çıkma Stratejisi Ölçeği verildi. Bulgular: Ebeveyn tutumları ve başa çıkma stratejileri açısından her iki grupta fark bulunmadı. Obez grupta yüksek oranda psikiyatrik hastalık saptandı. Ebeveynlerinin çocuklarıyla ilgili yaşam kalitesi algıları sağlıklı kontrollere göre düşük saptandı. Annede obezite açısından iki grup birbiri ile kıyaslandığında obez çocukların annelerinin daha yüksek oranda obez oldukları anlaşıldı. Sonuç: Anne babaların aksine çocukların kendileriyle ilgili yaşam kalitesi algıları sorunun daha az farkında olduklarını gösteriyordu. Bu da gelecekte gelişebilecek rahatsızlıklarla ilgili riskin daha fazla olabileceğini göstermektedir.
Abstract
Objective: This study aims to investigate the psychopathology, quality of life, perception and self-image of children and adolescents who were diagnosed with obesity; as well as attitudes, and coping strategies of their parents and their interrelationships. Methods: Our study was conducted on 30 patients aged 8 -18 years who applied to the Division of Pediatric Endocrinology of Department of Children’s Health and Disease of Akdeniz University with BMIs of 2 SD above the normal BMI, but without any history of mental retardation, and psychiatric consultations. Thirty age-, and gender-matched volunteered children, and adolescents aged 8-18 years without any medical disease or history of application to the outpatient clinics of pediatric psychiatry for any reason were included in the control group. Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present, and Lifetime Version was applied to all children, and adolescents. Rosenberg Self-Esteem Scale was used to evaluate their self-esteem. The Quality of Life Scale for Children was applied to the children and adolescents as well as their parents. To determine the methods of child rearing and coping strategies with stress, parents were given the Family Life and Child Rearing Demeanours Scale and the Coping Strategies Scale. Results: No differences were found between both groups regarding attitudes of parents, and their coping strategies. A higher percentage of psychiatric illnesses was found in the obese group. Parents’ perceptions of quality of life their children were inferior to others when compared with the healthy controls. When mothers of the two groups were compared, higher incidence of obesity was observed in the mothers of the obese children. Conclusion: Contrary to parents, perceptions of the children concerning their quality of life indicated that they were not aware of the problems as much as their parents which demonstrates that potential risk of development of problems in the future may be more dramatic.
