İntraoperatif Ultrason: Nöroşirürjikal Tümör Rezeksiyonlarında Kolay ve Maliyet Etkin Bir Metod

Özet
Amaç: Beyin tümörü tanısıyla ameliyat olan olgularda rezeksiyon oranı oldukça önemli bir prognostik faktör olarak kabul görmektedir. İntraoperatif ultrason kolay ulaşılabilir, ucuz, uygulaması kolay olan ve cerrahı ameliyat sırasında yönlendiren bir yöntemdir. Bu çalışmanın amacı, intraoperatif ultrason kullanılan olgularda kraniotomi sonrası tümöral kitlenin yerinin belirlenmesi, ultrasonografik görüntüsü ile patoloji arasındaki ilişkiyi araştırmak ve intraoperatif ultrason kullanımının rezidü kitlenin peroperatif olarak saptanmasındaki etkinliğini araştırmaktır. Yöntem: Beyin tümörü tanısı alıp 01.01.2014 ve 10.07.2018 tarihleri arasında Sağlık Bilimleri Üniversitesi İzmir Tepecik Eğitim ve Araştırma Hastanesi Nöroşirürji Kliniğinde bir cerrah (MÇ) tarafından ameliyat edilmiş ardışık 52 hastaya uygulanan 59 ameliyat retrospektif olarak tarandı. İntraoperatif ultrason kullanılmamış olan 25 hastanın 27 ameliyatı çalışma dışı bırakıldı. Yirmi yedi hastaya uygulanan 32 ameliyat çalışmaya dahil edildi. İntraoperatif ultrasonun lezyonu saptamadaki yararı, peroperatif olarak rezidü saptanan olgular ve patoloji sonuçları ile intra- operatif ultrason görüntüleri arasındaki ilişki araştırıldı. Bulgular: Lezyonu kistik komponent içeren hastalarda intraoperatif ultrason görüntü kalitesinin istatistiksel anlamlı olarak daha iyi olduğu saptandı (p<0,001). İntraoperatif ultrason görüntü kaliteleri değerlendirildiğinde patolojinin istatistiksel olarak anlamlı bir etkisinin olmadığı gözlendi (p=0,811). Tümörün supratentoriyel veya infratentoriyel yerleşimli olmasının intraoperatif ultrason görüntü kalitesini etkilemediği gösterildi (p=1). Sonuç: İntraoperatif ultrason kullanımının nöroşirürjikal tümör rezeksiyonlarında kolay ve maliyet etkin bir metod olduğu ve kullanımının giderek yaygınlaşacağı öngörülmektedir.
Abstract
Objective: Extent of resection is accepted as a significant prognostic factor in patients who had undergone surgery for brain tumors. Intraoperative ultrasound is an easily accessible, cost-effective real-time method that provides intraoperative guidance for the surgeon. The aim of this study is to determine the location of tumor after cranio- tomy with intraoperative ultrasound, to investigate both the correlation between ultrasonographic findings and the pathology results and also the efficacy of intraoperative ultrasound use in the detection of residual tumor perioperatively. Method: Fifty-nine operations performed by a single surgeon (MÇ) in consecutive 52 patients diagnosed with brain tumor in University of Health Sciences, Izmir Tepecik Education and Research Hospital Department of Neurosurgery between 01.01.2014 and 07.10. 2018 were retrospectively screened. Twenty-seven operations performed in 25 patients were excluded from the study because intraoperatively ultrasound was not used. Thirty-two operations performed for 27 patients were included in the study. The relationship among usefulness of intraoperative ultraso- und images in the identification of the lesion, the number of lesions with perioperatively detected residual tumors, their pathology results, and intraoperative ultrasonographic images were investigated. Results: The quality of intraoperative ultrasonographic images was found to be statistically significantly better in patients who had tumor with cystic component (p<0.001). It was also found that there was no statistically signifi- cant effect of pathology to the quality of intraoperative ultrasound image (p=0.811). The supratentorial or infra- tentorial location of the tumor did not affect the quality intraoperative ultrasound image (p=1). Conclusion: It has been envisaged that the use of intraoperative ultrasound is an easy and cost-effective method in neurosurgical tumor resections and its use will become increasingly widespread
