Hastane ve hastane dışı otopsi olgularında "bronşial mukus ve kalp kanında" postmortem olarak saptanan gram negatif çomakların sayısı ile morgda kalma sürelerinin karşılaştırılması

Özet

Çalışmamızda değişik ölüm sebepleriyle Adli Tıp Kuru¬mu morguna otopsi amacıyle gelen 60 hastane kökenli, 60 hastane dışı olmak üzere toplam 120 olgunun asepsi koşul¬larında antemortem steril olarak kabul edilen bronşial mu¬kus ve kalp kanı , materyalleri alınmış ve Gram negatif ço¬mak üremesi bakımından bakteriyolojik incelemeleri yapıl¬mıştır. Olguların morgda kaldıkları 1-3 gün boyunca değişik zaman aralıklarında alınan materyallerinde postmortem Gram negatif çomakların varlığını, saptanan çomakların cins ve tür düzeyinde tanısı ve en önemlisi morgda kalma süre¬leriyle üreme insidansları arasındaki ilişkiyi belirlemeye ça¬lıştık. Her iki materyalde üreme bakımından farklı araştırma grupları arasında bronşial mukus ve kalp kanında Gram ne¬gatif çomak üreme oranı bakımından anlamlı bir fark sapta-yamamıza (p>0.05) karşın, yüzde olarak hastane kökenli ol¬gularda daha fazla üreme belirlenmiştir. Hastane ve hastane dışı otopsi olgularının morgda kal¬ma süreleri içinde üreme insidansları karşılaştırıldığında has¬tane kökenlilerde alınan tüm materyallerde morg süresi uza¬dıkça üreme oranının azaldığı (p 0.05) saptanmıştır. Sonucumuz hastane kökenli otopsi olgularının yoğun bir bakteriyal kontaminasyona maruz kaldıklarını, hastane dışı otopsi olgularında ise mikroorganizma üreme ve yayılması¬na engel teşkil eden dokusal mikroorganizma florası ve ana¬tomik bölgelerin bozulmamasının önemini ortaya koymuş¬tur. Ayrıca her iki araştırma grubunda alınan bronşial mukus ve kalp kanı kültüründe en fazla Escherichia coli'nin izole edilmesi, olguların postmortem yoğun bir enterik bakteri kontaminasyonuna maruz kaldıklarını göstermiştir.

Abstract

Specimens obtained from bronchial mucus and heart blood of 120 cadavres who were sent to the morgue of Forensic Medicine Institution for necropsy are cultured for gram negative bacteria repeatedly for 1-3 days. 60 of the 120 cadavers were belonging to the hospitalized individuals with different death causes. The remaining cadavers were belonging to the individuals died out of hospital due to different causes. We tried to determine the occurrence, density and the genus and species distribution of the isolated gram negative bacilli. We also tried to determine the correlation between the growth rates and the time for which the cadavers kept in the morgue. The cadavers were separated into two groups according their origins. Although there was no significant difference between two groups regarding the bacterial growth rates in both of the specimens (p>0.05), the growth rates were slightly higher in the cadavers originated from the hospitals. This finding demonstrated the importance of the nosocomial microflora on the post-mortem bacterial contamination. The comparison of the growth rates in the relevance to the storage time in the morgue revealed that in contrast to the other group there was negative correlation between the storage time and bacterial growth rate in the hospitalized group (p<0.002). Our data showed that the hospitalized cases are exposed to a heavy bacterial contamination where as other necropsy cases belonging to non-hospitalized group have normal microflora because of preserved mucosal barrier. In addition the isolation of Escherichia coli as the predominating organism made evident the role of colonic flora in the postmortem bacterial contamination.
