Semptomatik Uterin Fibroidlerin Tedavisinde Uterin Arter Embolizasyonunun Uzun Dönemde Etkinliği ve Over Rezervine Etkisi

Özet
Amaç: Çalışmamızın amacı, semptomatik uterin fibroidlerin tedavisinde uterin arter embolizasyonunun etkinliğini ve over rezervine etkisini değerlendirmektir. Yöntem: Bu retrospektif kohort çalışmada, 1 Ocak 2010- 1 Ocak 2018 tarihleri arasında uterin arter embolizasyonu uygulanan 36 hastanın işlem öncesi ve işlemden sonra 6. ve 12. aylardaki verileri analiz edildi. Uterus ve fibroid volümü, kontrastlı manyetik rezonans görüntüleme ile değerlendirildi. Hastaların işlem öncesi ve sonrası menstürel kanama değişiklikleri ve hemoglobin düzeyleri izlendi. Ağrı yakınması, vizüel analog skalası ile değerlendirildi. İşlem öncesi ve sonrası over rezervi anti-müllerian hormon, folikül uyarıcı hormon ve estriol düzeyleri ile karşılaştırıldı. Sayısal veriler, tek yönlü ANOVA, katagorik veriler Mc Nemar testi ile analiz edildi. Bulgular: Ortalama yaş 42,0±5,8 (SD), takip süresi 62±7,1 (SD) (13-72) ay olarak hesaplandı. Hiçbir hastada per-op ve post-op komplikasyon gelişmez iken, yalnızca 1 hasta takipte işlem başarısızlığı nedeniyle cerrahiye verildi. İşlem sonrası 6. ve 12. aylarda, işlem öncesine kıyasla dominant fibroid (157,7, 72,8 ve 63,3 cm3 ) ve uterus hacminde (332,2, 265, 229 cm3 ) anlamlı küçülme izlendi (sırasıyla p<0,05, p<0,05 ve p<0,05, p<0,05). İşlem öncesi ve sonrası VAS skorlarındaki gerileme anlamlıydı (p<0,01, p<0,01). Keza, yoğun mestürel kanaması olan hasta sayısındaki azalma (%72,2, %33,3 ve %27,7) ve Hb düzeylerindeki artış (11,2±0,2, 12,3±0,1 ve 12,7±0,2 g/dL) anlamlı bulundu (p<0,01, p<0,01 ve p=0,04, p=0,03). İşlem sonrası takiplerde, over rezerv testlerinde anlamlı değişiklik gözlenmedi. Sonuç: Uterin arter embolizasyonu, semptomatik uterin fibroidlerin tedavisinde, düşük komplikasyon, yüksek etkinlik ve hasta memnuniyeti ile cerrahiye iyi bir alternatif olabilir. Uterin arter embolizasyonunun over rezervi üzerine olumsuz etkisi gösterilemedi.
Abstract
Objective: The aim of the study was to evaluate the efficacy of uterine artery embolization and its effects on ovarian reserve in the treatment of symptomatic uterine fibroids. Methods: In this retrospective cohort study, data of 36 patients who underwent uterine artery embolization between January 1, 2010 and January 1, 2018 were analyzed before and at 6 and 12 months after the procedure. Uterus and fibroid volume were evaluated by contrast-enhanced magnetic resonance imaging. Hemoglobin levels and menstrual bleeding changes were investigated before and after the procedure. Complaints of pain was evaluated using visual analog scale. Pre- and post-procedural ovarian reserve, anti-müllerian hormone, follicle stimulating hormone and estriol levels were compared. Numerical data were analyzed by one-way ANOVA and categorical data by Mc Nemar test. Results: The mean age was 42.0 ± 5.8 (SD) and the mean follow-up period was 62±7.1 (SD) (13-72) months. None of the patients developed peri- and post-operative complications. Only one patient was referred to surgery because of surgical failure. At postoperative 6 and 12 months,size of dominant fibroids (157.7, 72.8 and 63.3 cm3) and uterine volume (332.2, 265, 229 cm3) significantly decreased compared to preprocedural volumes (p<0.05, p<0.05 and p<0.05, p<0.05, respectively). The decrease in VAS scores before and after the procedure was significant (p<0.01, p<0.01). Also, decrease in both the number of patients with heavy mestural bleeding (72.2%, 33.3% and 27.7%) and in Hb levels (11.2±0.2, 12.3±0.1 and 12.7±0.2 g/dL) were significant (p<0.01, p<0.01 and p=0.04; p=0.03). No significant changes were observed in ovarian reserve tests during the postoperative follow-up. Conclusion: Uterine artery embolization may be a good alternative to surgery in the treatment of symptomatic uterine fibroids with low complications, high efficacy and patient satisfaction. The adverse effects of uterine artery embolization on ovarian reserve could not be demonstrated.
