KRONİK RİNOSİNÜZİTİ OLAN HASTALARDA ULUSLARARASI FRONTAL SİNÜS ANATOMİ SINIFLAMASINA GÖRE FRONTAL HÜCRELERİN ANALİZİ

Özet

Amaç: Çalışmamızda Uluslararası Frontal Sinüs Anatomi Sınıflamasına göre frontal hücrelerin kronik rinosinüzit (KrRNS) tanısı alan hastalarda görülme sıklığını ve bu hücrelerin frontal sinüzit ile ilişkilerini kontrol olgularla karşılaştırarak değerlendirilmesi amaçlanmaktadır. Yöntemler: Ocak 2014 ile Mart 2019 arasında KrRNS tan ısıyla tedavi planlanan ve tedavi öncesinde yüksek çözünürlüklü paranazal sinüs bilgisayarlı tomografisi (PNSBT) çekilmiş olgular ile hasta grubu oluşturuldu. KrRNS dışı endikasyonlar (kronik dakriosistit, başağrısı v.b.) nedeniyle PNSBT çekilmiş ve sinonazal patoloji izlenmeyen olgular kontrol grubuna dahil edildi. Bulgular: KrRNS'li 62 hastanın (124 taraf) ile 65 kontrol grubu olgusu (130 taraf) çalışmaya dahil edildi. Supra-bullar hücre (SBH) , supra-bullar frontal hücre (SBFH) ve frontal septal hücrelerin (FSH) KrRNS'li grupta kontrol grubuna göre anlamlı derecede daha sık olduğu gözlendi (p=0.002, p<0.0001, p=0.001; s ırayla). SBH, SBFH ve FSH ile frontal sinüzitin insidansı ve şiddetiyle de pozitif korele olduğu izlendi. Sonuç: SBH, SBFH ve FSH gibi hücrelerin frontal sinüzit gelişiminde rol oynayabileceği akılda tutulmalıdır. Cerrahi tedavinin başarı şansını arttırmak için tedavi planında bu hücrelerin göz önünde bulundurulması gereklidir.

Abstract

Objective: The goal of this study was to evaluate the prevelance of frontal cells (according to International Frontal Sinus Anatomy Classification) in patients with chronic rhinosinusitis (CrRNS) and to compare them with the control group and assess the association with frontal sinusitis. Methods: The patients who were treated with the diagnosis of CrRNS between January 2014 and March 2019 and underwent high resolution paranasal sinus computed tomography (PNSCT) were included in the study. The subjects without sinonasal disease who had other indications (chronic dacryocystitis, headache etc.) for PNSCT were determined as control group. Results: 62 patients (124 sides) with CrRNS and 65 patients (130 sides) in the control group were included in the study. Supra-bullar cells (SBC), supra-bullar frontal cells (SBFC) and frontal septal cells (FSC) were significantly more frequent in patients with CrRNS than the control subjects (p=0.002, p<0.0001, p=0.001, respectively). The presences of SBC, SBFC and FSC were significantly correlated positively with the incidence and severity of frontal sinusitis. Conclusion: It should be kept in mind that cells such as SBC, SBFC and FSC may play a role in the development of frontal sinusitis. These cells may need to be considered in the treatment plan in order to increase the success rate of surgical treatment.
