Özet

Giriş: Nedeni bilinmeyen ateş (NBA) etiyolojisinde enfeksiyonlar ilk sırada yer almakla birlikte; maligniteler, bağ doku hastalıkları, ilaç kullanımı gibi diğer nedenler ve tanı konulamayan hasta gruplarıda yer almaktadır. NBA sebepleri coğrafi bölgelere, ülkelerin gelişmişlik düzeyine, hastanın yaşına, hekimin deneyimine ve hastane olanaklarına göre değişebilmekte olup bu çalışmada hastanemize başvuran NBA olgularının; etiyoloji, klinik, laboratuvar bulguları ve prognoz açısından retrospektif olarak değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmaya hastanemiz enfeksiyon hastalıkları kliniğinde Ocak 2009 - Aralık 2015 tarihlerinde takip edilen, NBA tanısı almış olan 18 yaş ve üzeri 306 hasta dahil edilmiş olup; HIV pozitif, nötropenik hastalar ve nozokomiyal NBA olguları çalışma dışı bırakılmıştır. Hastalar etiyoloji, klinik, laboratuvar bulguları ve prognozlarına yönelik olarak retrospektif olarak incelenmiştir. Bulgular: Çalışmamızda NBA’nın en sık nedeni 169 hasta (%55.2) ile enfeksiyon olarak bulundu. Enfeksiyonlar içerisinde136 (%44.4) hastada bakteriyel etkenler ve bunun içinde de25 (%8.1) hasta ile tüberküloz ilk sırada tespit edildi. Enfeksiyonları; 44 (%14.4) olgu ile tanı konulamayan grup,39 (%12.7) olgu ile diğer nedenler, 33 (%10.8) olgu ile maligniteler ve 21 (%6.9) olgu ile bağ doku hastalıklarının takip ettiği görüldü. Sonuç: NBA tanısı konulan hastalarda etiyoloji incelendiğinde enfeksiyonlar halen ilk sırada görülmektedir. Ülkemizde; bu enfeksiyonlar içinde tüberküloz, bruselloz ve enfektif endokardit ilk sıralarda yer almaktadır. NBA olgularının tanısında standart bir yaklaşım mümkün olmayıp; hekimin deneyimi, hastanenin tanısal olanakları ve o bölgede sık görülen etkenler önemlidir. Bu nedenle etiyolojiye yönelik tetkiklerin planlamasında hastanın yaşı, coğrafi bölge ve ülkelerin gelişmişlik düzeyi göz önüne alınmalı; mortalitesi yüksek olan hastalıklar atlanmamalı; ülkemiz için endemik olan tüberküloz ve bruselloz mutlaka akla gelmelidir.

Abstract

Introduction: Although infections are the first in the etiology of fever of unknown origin (FUO); other causes such as malignancies, connective tissue diseases, medication use, and undiagnosed patient groups are also included The reasons for FUO may vary according to geographical regions, development level of countries, age of patient, experience of physician and hospital facilities. The aim of this study was to evaluate the etiology, clinical, laboratory findings and prognosis retrospectively. Material and Method: The study included 306 patients aged 18 years and older who were diagnosed with FUO in the infectious diseases clinic of our hospital between January 2009 and December 2015; HIV-positive, neutropenic patients and nosocomial NBA cases were excluded from the study. The patients were retrospectively analyzed for etiology, clinical, laboratory findings and prognosis. Results: The most common cause of FUO in our study was infection with 169 patients (55.2%). Among the infections, bacterial agents were detected in 136 (44.4%) patients and tuberculosis was found in the first order with 25 (8.1%) patients. Infections; 44 (14.4%) cases were diagnosed with undiagnosed group, 39 (12.7%) cases were caused by other causes, 33 (10.8%) cases were malignancies and 21 (6.9%) cases followed by connective tissue diseases. Conclusion: When the etiology is examined in the patients with FUO, the infections are still seen in the first order. In our country; these infections include tuberculosis, brucellosis and infective endocarditis. There is no standard approach in the diagnosis of FUO cases; the experience of the physician, the diagnostic possibilities of the hospital and the common factors in the region are important. Therefore, the age of the patient, geographic region and the level of development of countries should be taken into consideration in the planning of the etiology tests; diseases with high mortality should not be missed; tuberculosis and brucellosis, which are endemic to our country, should be kept in mind.
