PAP TEDAVİSİ UYUMSUZLUĞU OLAN OSAS HASTALARININ YAŞ, CİNSİYET, BMİ VE AHİ'LERİNİN KARŞILAŞTIRILMASI

Özet

Amaç: Orta ve ağır derecede uyku apnesinde standart tedavi PAP (Positive Airway Pressure) tedavisidir. Cerrahinin riskli olduğu kardiak ve serebrovasküler hastalığı olan hafif dereceli uyku apnesi olan hastalarda da PAP tedavisi önerilmektedir. Bu çalışmanın amacı PAP tedavisi uyumsuzluğu olan OSAS hastalarının yaş, cinsiyet, BMI (body mass index: vücut kitle indeksi), önerilen cihaz tipi , ek hastalık varlığı (diyabet, hipertansiyon, kronik obstrüktif akciğer hastalığı, hiperlipidemi, iskemik kalp hastalığı) , septum deviasyonu varlığını AHİ (apne-hipopne indeksi) ile karşılaştırarak ilişkisini araştırmaktır. Yöntem ve Gereçler: 1 Ocak 2018- 1 Ocak 2019 tarihleri arasında XXXX Şehir Eğitim ve Araştırma Hastanesi KBB kliniğine müracaat eden, uyku laboratuarında polisomnografi (Grass Comed Plus, UK) ile obstrüktif uyku apnesi tanısı konulan ve PAP tedavisi (CPAP (continuous positive airway pressure) , BPAP (bilevel positive airway pressure) , BPAP-ST (bilevel positive airway pressure-spontaneous timed) , APAP (automatic positive airway pressure) önerilen fakat çeşitli nedenlerden dolayı (sosyoekonomik nedenler, maske ve cihaz ile ilgili nedenler) PAP tedavisi uyumsuzluğu olan 50 OSAS'lı hastanın dosyaları retrospektif olarak araştırıldı. Bulgular: Hafif, orta ve ağır OSAS grupları arasında AHİ ile yaş ve vücut kitle indeksi arasında istatistiksel olarak anlamlı ilişki yoktu (p=0.294) . Cinsiyete göre OSAS grupları arasında anlamlı bir fark yoktu (p=0.064) . Gruplar arasında cihaz tipiyle AHİ ortalamaları arasında anlamlı bir fark yoktu (p=0.221) . Ek hastalık varlığına göre grupların AHİ ortalamaları arasında anlamlı bir fark yoktu (p=0.564) . Ayrıca 3 grupta nazal septum deviasyonu olan hastaların AHİ ortalamaları arasında da anlamlı bir fark bulunamamıştır (p=0.548). Sonuç: PAP uyumsuzluğu olan OSAS'lı hastaların yaş, cinsiyet, BMI, AHİ, cihaz tipi, ek hastalık ve septum deviasyonu faktörlerinin hastalığın derecesiyle ilişkisi bulunamamıştır. Daha çok hasta sayısı olan ileri çalışmalara ihtiyaç vardır.

Abstract

Objective: Positive airway (PAP) is the standart treatment for moderate and severe sleep apnea. PAP treatment is also recommended in patients with mild sleep apnea with cardiac and cerebrovascular disease, where surgery is risky. The aim of this study was to determine the age, gender, BMI, recommended device type, presence of additional disease (diabetes, hypertension, chronic obstructive pulmonary disease, hyperlipidemia, ischemic heart disease) , septum deviation with AHI in OSAS patients with noncompliace to PAP treatment. Material and Methods: Between January 1st, 2018 and January 1st, 2019 the patients admitted to XXXXX City Training and Research Hospital Ear-Nose-Throat clinic who were diagnosed obstructive sleep apnea by polysomnography (Grass Comed Plus, UK) in our sleep laboratory with non compliance to PAP treatment (CPAP (continous airway pressure) , BPAP (bilevel positive airway pressure) , BPAP-ST (standart time level), APAP (automatic positive airway pressure). Results: There was no statistically significant relationship between AHI values and the age or BMI of mild, moderate and severe OSAS groups (p=0.294) . There was no significant difference between OSAS groups according to gender (p=0.064) . There was no significant difference between the device type and AHI (p=0.021) . There was no significant difference between the mean AHI values of the groups according to the presence of additional disease (p=0.564) . In addition no significant difference was found between the mean AHI values of patients with nasal septun deviation in 3 groups (p=0.548). Conclusion: Age, gender, BMI, PAP device type, comorbidity and septum deviation were not correlated with the degree of the disease in OSAS patients with noncompliance to PAP treatment. Further studies with more patients are needed.
