Klasik Fenilketonüri Tanılı Olgularda Fenilalaninden Kısıtlı Diyetin Antropometrik Ölçümler Üzerine Etkisi

Özet

Amaç: Klasik fenilketonüri (FKÜ) doğumsal bir amino asit metabolizma bozukluğu hastalığıdır. Erken tedavinin hastalığın prognozunu etkileyen en önemli faktör olması nedeniyle hastalık ülkemizde olduğu gibi birçok ülkede yenidoğan tarama programıyla taranmaktadır. Tedavinin temelini, yaşam boyu fenilalaninden kısıtlı diyet oluşturur. Bu çalışmada, fenilketonüri tanılı olgularda fenilalaninden kısıtlı diyetin antropometrik ölçümler üzerine etkisinin araştırılması amaçlanmıştır. Yöntem: Bu çalışma, Çocuk Beslenme ve Metabolizma Polikliniğinde, 2007-2015 yılları arasında, diyet tedavisi ile izlenen FKÜ tanılı 40 hasta ile diyet tedavisi almayan hafif hiperfenilalaninemi (HFA) tanısı ile izlenen 78 hastanın dosyalarının geriye dönük olarak incelenmesiyle yapıldı. Hastaların doğum, yaşamın 6., 12., 18., 24., 30. ve 36. ayında bakılan vücut ağırlığı, boy, vücut ağırlığı standart deviasyon skoru (SDS), boy SDS ve vücut kütle indeksi (VKİ) SDS değerlendirildi. Bulgular: Olguların doğum, 6., 12., 18., 24., 30. ve 36. ay ortalama vücut ağırlıkları karşılaştırıldığında, iki grupta kilo alımı açısından fark saptanmadı (p=0,516). Altı ay arayla bakılan boy uzunlukları incelendiğinde, üç yılın sonunda boy uzama hızının HFA grubunda FKÜ grubuna göre yüksek olduğu belirlendi (F(1,33)=3,443, p=0,037). Olguların doğum, 6. ay, 12. ay, 18. ay, 24. ay, 30. ay ve 36. ay vücut ağırlığı SDS, boy SDS ve VKİ SDS karşılaştırıldığında, iki grup arasında istatistiksel fark saptanmadı. Sonuç: Sonuç olarak, diyet tedavisi ile izlenen FKÜ hastalarında yaşamın ilk üç yılında HFA hastalarına göre boy uzama hızının yavaş olduğu fakat iki grup arasında VKİ açısından anlamlı fark olmadığı görülmektedir. Klasik fenilketonüri hastalarında boy uzamasının yavaş olmasına hastalara uygulanan fenilalaninden ve doğal proteinden kısıtlı diyetin neden olduğunu düşünmekteyiz.

Abstract

Objective: Classical phenylketonuria (PKU) is an inherited disorder of amino acid metabolism. Since treatment at an early stage is the most important factor effecting prognosis of the disease, the disease is screened with newborn screening program in our and in many countries as well. The basis of treatment is a life-long phenylalanine-restricted diet. In this study, we aimed to evaluate the effect of phenylalanine - restricted diet on anthropometric parameters of patients with the diagnosis of phenylketonuria Methods: This study was conducted with retrospective review of the charts of 40 patients with PKU followed by dietary therapy and 78 patients with mild hyperphenylalaninemia (HPA) without dietary therapy in a pediatric nutrition and metabolism clinic between 2007 and 2015. Weight, height, weight standard deviation score (SDS), height SDS, Body mass index (BMI) SDS of the patients were assessed at birth, 6th, 12th, 18th, 24th, 30th, 36th months of life. Results: In the comparison of the mean body weights of the patients at birth. 6th,12th, 18th, 24th, 30th, 36 months of life, there was no difference in weight gain of both groups (p=0.516). In the evaluation of height at six month- intervals, , height growth rate was higher in HPA group than PKU group at the end of the 3 years of life (F(1,33)=3.443,p=0.037). When body weights, height SDS, and BMI SDS were compared of the cases determined at birth 6th, 12th, 18th, 24th, 30th, 36th months of life any significant difference between the two groups was not detected Conclusion: We can conclude that the height growth rate is lower in PKU patients followed up by dietary therapy than HPA patients, without any significant intergroup difference as for BMI We think that the lower height growth rate of PKU patients is the result of phenylalanine - and neutral protein-restricted diet.
