Özet
Amaç: Portal hipertansif çocuklarda hipoksemiye yol açan ve hepatopulmoner sendrom açısından uyarıcı olabilecek solunum fonksiyon bozukluklarını belirlemek amaçlanmıştır. Yöntem: Endoskopik skleroterapi programındaki 16 olgunun solunum fonksiyonları incelendi. Bu amaçla; arter kan gazı, akciğer hacmi, spirometrik değerler, difüzyon kapasitesi ve maksimum solunum basınçları değerlendirildi. Bulgular: Solunum fonksiyon bozukluğu göstergesi olarak; üç olguda hipoksemi ve iki olguda ortodeoksi belirlendi. Akciğer hacimleri normal sınırlarda iken maksimum solunum basınçları ve difüzyon kapasiteleri düşük bulundu. Sonuç: Solunum sıkıntısı olan ya da olmayan tüm portal hipertansif olgularda solunum sistemi değerlendirilmelidir. Bu yaklaşımla birincil solunum bozuklukları belirlenir. Aynı zamanda, solunum fonksiyon testlerinin iki parçası olan arter kan gazı incelemesi ve difüzyon kapasitesi, hepatopulmoner sendroma yönelik detaylı araştırmaların gerekliliğini ortaya koymada ilk belirleyicilerdir.
Abstract
Aim: To determine the pulmonary functional defects, which may contribute to hypoxemia and warn about the hepatopulmonary syndrome, in children with portal hypertension Methods: The pulmonary function of 16 portal hypertensive children already on an endoscopic sclerotherapy program, were evaluated. Arterial blood gases, lung volumes, spirometric values, diffusing capacity and maximal respiratory pressures were investigated as pulmonary function tests. Results: Three children had hypoxemia and two had orthodeoxia suggesting deterioration in pulmonary function. The lung volumes were in normal ranges but the maximal respiratory pressures and the diffusing capacities were found to be decreased in the children, in whom the tests were effectively applied. Conclusion: Thorough pulmonary evaluation has to be performed in all portal hypertensive children with or without pulmonary symptoms. With this approach, not only the primary pulmonary disorders are eliminated but also two components of pulmonary function testing - arterial blood gas analysis and diffusion test - can be used as the first step predictive factors for more detailed research protocol of hepatopulmonary syndrome.
