İmmünglobulin A Eksikliğinde Atopik Hastalıkların Sıklığı

Özet
Amaç: Selektif IgA eksikliği en sık rastlanan immünglobulin bozukluğudur. IgA eksikliği klinikte değişik tablolar ile prezante olabilmektedir. Asemptomatik seyredebildiği gibi otoimmün ve atopik hastalıklara eşlik ettiği belirlenmiştir. Çalışmamızda IgA eksikliği ve atopi ilişkisini saptamak ve Ig A eksikliği olan olgularda atopi sıklığının araştırılması amaçlanmıştır. Yöntem: Dokuz Eylül Üniversitesi Tıp Fakültesi Çocuk Alerji Bilim Dalı, Çocuk Gastroenteroloji Bilim Dalı ve Genel Çocuk Polikliniğine başvuran ve herhangi bir nedenle bakılan Ig A düzeyi yaşına göre -2 SD altında saptanan 4-18 yaş arası 96 hastaya ulaşıldı. Veriler hastane kayıt sisteminden retrospektif olarak tarandı. Anne veya babalara telefon aracılığıyla ulaşılarak Ig A eksikliği ve atopi ilişkisi sorgulanmak üzere International Study of Asthma and Allergies in Childhood (ISAAC) çalışma sorularını içeren anket çalışması yapıldı. IgA eksikliğinde alerjik rinit, astım, egzama, gıda alerjisi sıklığı bakıldı. Bulgular: Hastaların 56’sı (%58,3) erkek, 40’ı (%41,7) kızdı. Ortalama yaş 7,4±3,1 yıldı. Hastaların %26’sı astım, %16,7‘sinde atopik dermatit, %9,4’ünde gıda alerjisi, %7,3’ünde alerjik rinit, %6,3‘ünde egzama, %2,1’inde ilaç alerjisi mevcuttu. Olgularımızın %81,3’ünde deri testi ve kanda spesifik alerji testleri yapılmış olup alerjen duyarlılığı %18,8 olarak değerlendirildi. Sonuç: IgA eksikliği olan çocuklarda geçmişte hırıltılı solunum yakınması, astım, atopik dermatit, gıda alerjisi, normal popülasyonda yapılan araştırma sonuçları ile karşılaştırıldığında daha sık olduğu saptanmıştır.
Abstract
Objective: Selective IgA deficiency is the most common immunoglobulin disorder. IgA deficiency may have different clinical presentations. While some patients are asymptomatic it has been also detected in association with autoimmune and atopic disorders. In our study, we aimed to determine the relationship between Ig A deficiency and atopy and the prevalence of atopy in patients with IgA deficiency. Method: Ninety-six patients between the ages of 4 and 18, who were admitted to the Dokuz Eylül University Faculty of Medicine, Department of Pediatric Allergy, Department of Pediatric Gastroenterology and General Pediatric Outpatient Clinic, and whose IgA levels were below-2 SD tested for any reason, were reached. Data were scanned from hospital data management system retrospectively. A questionnaire including questions from International Study of Asthma and Allergies in Childhood (ISAAC) study, related to IgA deficiency and atopy was formulated by reaching the parents on phone. We investigated the frequency of asthma, allergic rhinitis, asthma, eczema and food intolerance in IgA deficiency. Results: Fifty-six (58.3%) patients were male and 40 (41.7%) were female. The mean age was 7.4±3.1 years . Twenty six percent of the patients The patients had asthma (26%), allergic rhinitis (7.3%), atopic dermatitis (16.7%), food allergy (9.4%), allergic rhinitis (7.3%), eczema (6.3%), and drug allergy (2.1%). Skin test and specific allergy tests in blood samples were performed in 81.3% of our patients and allergen sensitivity was found in 18.8% of the patients. Conclusion: In children with Ig A deficiency, wheezing complaints asthma, atopic dermatitis, food allergy were detected to be more common in comparison with the results of the survey conducted in the normal population.
