Alt Ekstremite Selüliti Gelişiminde Risk Faktörlerinin İncelenmesi

Özet
Giriş: Retrospektif olgu-kontrol çalışmasında, servisimizde izlenen alt ekstremite yerleşimli selülit olgularının demografik ve epidemiyolojik özelliklerinin saptanması, selülit gelişimi ve morbidite ile ilişkili risk faktörlerinin ortaya çıkarılması amaçlandı. Materyal ve Metod: Bu çalışmaya Sağlık Bilimleri Üniversitesi Samsun Eğitim ve Araştırma Hastanesi İnfeksiyon Hastalıkları ve Klinik Mikrobiyoloji Kliniğinde 1 Ocak 2017-31 Aralık 2017 tarihleri arasında alt ekstremite selüliti tanısıyla yatırılarak tedavi edilmiş olan 54 ve alt ekstremite selüliti tanısı olmayan 54 kişi dahil edildi. Hastalarda bu infeksiyonun gelişimi açısından risk faktörlerinin belirlenmesi ve epidemiyolojik değerlendirmelerinin yapılması amacıyla tinea pedis, onikomikoz, alt ekstremitede venöz yetersizlik, önceden geçirilmiş selülit hikayesi, diabetes mellitus ve sigara kullanımı retrospektif dosya incelemesi yapılarak araştırıldı. Bulgular: Çalışmamızda ekstremite selüliti gelişimi için risk faktörleri olarak; tinea pedis (p= 0.001), onikomikoz (p= 0.001), alt ekstremitede venöz yetersizlik (p= 0.021), önceden geçirilmiş selülit hikayesi (p= 0.001), diabetes mellitus (p= 0.001) ve sigara kullanımı (p= 0.001) bulundu. Sonuç: Alt ekstremite selüliti gelişimindeki önlenebilir risk faktörlerinin ortadan kaldırılmasıyla, geniş spektrumlu antibiyotik kullanımı, eczane maliyetleri ve toplam hastanede yatış maliyetleri azalacaktır.
Abstract
Introduction: In this retrospective case-control study, it was aimed to identify demographic and epidemiologic features of patients with cellulitis of the lower-extremity as well as the related risk factors regarding morbidity. Materials and Methods: This study included patients (54 patients) who were hospitalized with lower extremity cellulitis between January 1, 2017 and December 31, 2017 in University of Health Sciencies Samsun Training and Research Hospital, Infectious diseases and Clinical Microbiology Clinic, and a control group of 54 patients without lower extremity cellulitis. Tinea pedis, onychomycosis, lower extremity venous insufficiency, previous history of cellulitis, diabetes mellitus, and smoking were investigated in order to determine the risk factors for the development of this infection and to make epidemiological evaluations in these patients by retrospective file examination. Results: In our study, according to statistical analysis regarding the development of cellulitis of the lower-extremity; tinea pedis (p= 0.001), onychomycosis (p= 0.001), chronic venous disease of the lower limbs (p= 0.021), pre-existing cellulite (p= 0.001), diabetes mellitus (p= 0.001), and smoking (p= 0.001) were observed to be statistically significant. Conclusion: The abolition of preventable risk factors in the development of lower extremity cellulite will reduce the use of broad spectrum antibiotics, pharmacy costs and total hospitalization costs.
