Özet

Giriş: Bruselloz kronik seyredebilen ve komplikasyonları yaygın olarak gözlenebilen zoonotik bir hastalıktır. Brucelllae türlerinin fagositler içinde yaşayayıp çoğalabilen fakültatif intraselüler mikroorganizmalar olmalarıtedavide kullanılan antimikrobiyallere karşı bir miktar korunma sağlar bu yüzden in vitro olarak birçok antibiyotiğe duyarlı olmalarına karşın hastalığın tedavisinde güçlükler yaşanmaktadır. Bu çalışmada bruselloz tedavisinde kullanılan antibiyotiklerin duyarlılıklarının belirlenmesi, MİK50 ve MİK90 değerlerinin tespiti ve güncel tedavilerin yeterliliğinin gözden geçirilmesi amaçlanmıştır. Gereç ve Yöntem: Ocak 2003-Ekim 2013 tarihleri arasında hastaların kan kültürlerinden izole edilen 120 Brucella suşunun rifampisin, streptomisin, tetrasiklin ve ciprofloksasin için minimal inhibitör konsantrasyonları (MİK) E-test stripleri (AB Biodisk, İsveç) kullanılarak araştırıldı. Bulgular: The Clinical and Laboratory Standards Institute (CLSI)'nın Brucella spp. için belirlediği sınır değerlere göre suşların tümü streptomisin duyarlı, bir suş tetrasiklin dirençli saptandı. CLSI’nın güç üreyen bakterilerden olan Haemophilus türleri için belirlediği sınır değerler dikkate alındığında bir suş siprofloksasine dirençli, altı suşrifampisine orta düzey dirençli olarak saptandı. Sonuç: Sonuçlar dünyada ve ülkemizde yapılan çalışmalarla uyumlu bulundu.

Abstract

Introduction: Brucellosis is a zoonotic infectious disease that may become chronic and have various complications. The brucelllae are facultative intracellular pathogens that can survive and multiply within phagocytes and this intracellular localization offers some protection against antimicrobials. So, although Brucella spp. are susceptible to many antibiotics in vitro, difficulties in clinical treatment of the disease may be seen. In this study we aimed to analyse the susceptibility of antibiotics commonly used for the treatment of Brucellosis and to deterimine the minimal inhibitory concentrations (MIC50 MIC90). Material and Method: Between January 2003-October 2013, 120 isolates of Brucella spp. from blood cultures were tested for MIC’s for rifampisin, streptomisin, tetracycline ve ciprofloxacin by using E-test (AB Biodisk, Sweeden). Results: All isolates were found as suscetible to streptomycine but one isolate was resistant to tetracycline according to The Clinical and Laboratory Standards Institute (CLSI) criterias in susceptibility testing of Brucella spp. Considering the limit values that CLSI specifies for species of Haemophilus, a hard growing bacterium, one isolate was resistant to ciprofloxacin and six isolates were intermediately resistant to rifampicin. Conclusion: Our findings were consistent with the results of other studies from the world and from our country.
