Özet
Konstrüktif perikardit ender görülen ancak diyastolik doluşlarının bozulmasına neden olarak ciddi hasar oluşturabilen klinik bir sendromdur. Bu makalede geç tanı almış konstrüktif perikardit nedeniyle siroz tanısı konan bir olgunun perioperatif anestezi öyküsünü sunmayı amaçladık. Kırk dokuz yaş, 70 kg erkek hasta, daha önce bilinç kaybı, nefes darlığı, sarılık nedeniyle hastaneye kaldırıldı. Periferik ödem, asit, gastrointestinal kanama, hepatomegali, ensefalopati, antiHCV pozitifliği nedeniyle hastanın HCV sirozu + hepatik ensefalopati teşhisi kondu. Yenilenen tahlillerde HCV negatifti ve tanı tekrar gözden geçirildi. Kardiyolojik muayenesinde konstriktif perikardit tanısı alıp, ameliyat kararı verildi. Hasta apatik, kısmen oryante, Elektroensefalogram (EEG)'da orta derecede yavaşlama ile yüksek nörolojik riski altında tutuldu. Anestezi indüksiyonu öncesi kan basıncı 114/60, kalp hızı 95, sağ-sol near infrared spektroskopi (NIRS,rSO2) 43/45, patient state index (PSI) 95, SpO2 97. Anestezi indüksiyonunda, ketamin, midazolam, lidokain, fentanil, rokuronyum kullanıldı. İndiksiyonla birlikte transamin ve dobutamin infüzyonu başlatıldı. İndüksiyondan sonra sağ-sol rSO2 57/59, PSI 41, spektral kenar frekansları (SEF) 6.6 / 6,5, santral venöz basınç (CVP) 26. Transözofageal ekokardiyografi (TEE), özofagus varisleri nedeniyle yapılmak istenmedi. Ameliyat sırasında milrinon infüzyonu başlatıldı. Postoperatif 4 saat sonra ekstübe edildi, inotrop infüzyonları ise 2 gün sonra sonlandırıldı. Sol kalp pulmoner venöz dönüşü inspirasyon esnasında azalır. Bu, interventriküler septumu sola kaydırır ve sol ventrikül kavitesi ve strok volüm azalır. Strok volümdeki düşüşe verilen en önemli refleks yanıt adrenerjik uyarılmadır. Bu nedenle perioperatif dönemde adrenerjik uyarımı korumak önemlidir. Barorefleks duyarlılığında azalma da kardiyak sirozun bir bileşeni olarak gösterilmiştir. Bu nedenle anestezi indüksiyonunun neden olabileceği adrenerjik depresyonu önlemek için indüksiyon ajanları ketamin ve dobutamin ile kombine edilmiştir.
Abstract
Constrictive pericarditis is a rare clinical syndrome that can cause severe worsening of the diastolic filling. In this article, we aimed to present the perioperative anesthesia story of a case with a delayed diagnosis of cirrhosis due to constructive pericarditis. A 49 yearold, 70 kg male patient had been admitted to a hospital due to loss of consciousness, shortness of breath, and jaundice. He had peripheral edema, ascites, gastrointestinal bleeding, hepatomegaly, encephalopathy, antiHCV positivity,so the patient was diagnosed as having HCV cirrhosis+hepatic encephalopathy. HCV was negative in the renewed examinations, and the diagnosis was reconsidered On cardiology examination, he was diagnosed as constrictive pericarditis and decision of operation was made. Patient was operated on with high risk of neurological impairment with apathetic, slow-sense and responsive, partially oriented, moderate deceleration on electroencephalograms (EEG). Before anesthesia induction, blood pressure (114/60), heart rate (95 bpm), right-left near infrared spectroscopy (NIRS, rSO2 ) (43/45),patient state index (PSI) (95), SpO2 (97) were recorded as indicated.. During induction of anesthesia ketamine, midazolam, lidocaine, fentanyl, and rocuronium were used. Meanwhile, transamine and dobutamine infusions were initiated concurrently with induction. After induction, rightleft rSO2 (57/59), PSI (41), SEF (6.6/6.5). CVP (26) were measured. TEE was not performedbecause of the presence of esophageal varices. Intraoperatively, infusion of milrinone was initiated. After 4 hours he was extubated, infusions were terminated after 2 days. The left heart pulmonary venous return decreases during inspiration which shifts interventricular septum to the left and reduces left ventricular cavity so that stroke volume is reduced. The most important compensatory reflexive response to decrease in stroke volume is adrenergic stimulation. So it is important to maintain adrenergic stimulation during perioperative period. A decrease in baroreflex sensitivity has been shown as a component of cardiac cirrhosis also.Therefore, to prevent adrenergic depression that may be caused by anesthesia induction, induction agents were combined with ketamine and dobutamine.
