Özet

Amaç: Kantitatif butirilkolin esteraz (BChe) düzeyi tayinine ek olarak preoperatif genotip analizinin sağlıklı gönüllüler ile kıyaslandığında BChe ile yıkılan kas gevşeticilerin kullanıldığı genel anestezi ile elektif sezaryen planlanan gebelerde yararlı olup olmayacağını araştırmayı amaçladık. Yöntem: Etik kurul onayını takiben araştırmaya katılmaya onay veren gebe olan ve olmayan sağlıklı gönüllülerden (kontrol) toplanan 10 ml periferik venöz kan örnekleri biyokimyasal ve genetik analiz için -20°C’de saklandı. Daha sonra BChe enzim düzeyleri ölçüldü ve kan örneklerinden DNA elde edildi. Primerlere (İleri primer 5’-TTCAAAAAGCCACAGTCTCT-3’ve Geri primer 5’-GATATAAACAGTCTTCACTG-3’) termal döngü cihazında PCR işlemi takiben genotipleme yapıldı (UU= 81, 51, 25 homozigot yabanıl tip, UA= 106, 81, 51, 25 heterozigot, AA= 106, 50 bp homozigot mutant tip). Genotip ve enzim sonuçları karşılaştırılarak, genotiplerin klinik bulgulara katkısı değerlendirildi. Bulgular: Otuz iki gebede UA - heterozigot genotip ve 10 gebede UU-yabanıl genotip saptanırken, sağlıklı kontrollerden 24’ünde UA-heterozigot genotip ve 3’ünde UU-yabanıl genotip olduğu belirlendi. Her iki grupta da AA-homozigot mutant genotip saptanmadı. Sonuç: BChe ile hidrolize olan kas gevşeticilerle uygulanan genel anestezi altında elektif sezaryen operasyonu geçirecek ve kas gevşeticilere bağlı preoperatif apne öyküsü olan gebelerde preoepratif BCHe enzim düzeyleri ile birlikte genotip çalışması yapılması anestezi güvenliğinin artmasına yardımcı olabilir. BChe enzimine ait AA homozigot genotipli gebe popülasyonunda karşılaşma olasılığı nedeniyle preoperatif genotip belirleme testlerinin rutin olarak yapılabilmesi için daha büyük taramalara gereksinim vardır.

Abstract

Objective: We aimed to investigate whether preoperative genotype analysis in addition to determination of quantitative butyrylcholinesterase (BChe) levels is useful or not in parturients scheduled for elective cesarean delivery under general anesthesia using muscle relaxants metabolized by BChe when compared to non-anesthetized healthy volunteers. Method: After obtaining ethics committee approval, 10 mL of venous blood samples collected from pregnants and non-pregnant healthy volunteers (control) were kept at -20°C for biochemical and genetic analysis. Afterwards BChe enzyme levels were determined and DNAs were obtained from blood samples. After PCR processing for the primaries (forward primer 5’-TTCAAAAAGCCACAGTCTCT-3 and backward primer 5’-GATATAAACAGTCTTCACTG-3’) in the thermal cycler, genotyping was performed (UU= 81, 51, 25 homozygot type, UA= 106, 81, 51, 25 heterozygot, AA= 106, 50 bp homozygot mutant type). Contribution of genotypes to the clinical findings has been evaluated by comparing genotype and enzyme results. Results: UA heterozygot type (n= 32 pregnants) and UU wild type genotype (n=10 pregnants) were observed, while UA heterozygot genotype and UA wild type were found in 24 and 3 healthy volunteers, respectively. No AA homozygote mutant genotype has been found in this study population. Conclusion: Preoperative determination of BChe enzyme levels and genotyping together might aid the safety of anesthesia, when parturients with a history of postoperative apnea due to muscle relaxants are scheduled to undergo elective cesarean operation under general anesthesia using neuromuscular blockers that are hydrolyzed by BChe. Larger size testing is needed in order to make genotype determination tests in a routine manner since there is a probability of encountering AA homozygote genotype belonging to Bche enzyme.
