PAROTİS BEZİ KİTLELERİNE YAKLAŞIM: KLİNİK SONUÇLARIMIZ

Özet
Amaç: Parotis bezi kitlesi nedeniyle opere ettiğimiz hastaların klinik özelikleri, cerrahi tedavi yöntemleri, histopatolojik sonuçları ve komplikasyonlarını değerlendirmeyi amaçladık. Yöntem: Parotis bezinde kitle nedeniyle 2011-2019 yılları arasında kliniğimizde opere edilen 93 hastanın klinik verileri retrospektif olarak incelendi. Bulgular: Hastaların 54’ü (%59.2) erkek, 39’u (%40.8) kadındı. Hastaların yaş ortalaması 44.17±13.20 idi. 81 hastada (%92.4) kulak önünde ağrısız şişlik nedeni ile poliklinik başvurusu gerçekleşti. Tümör hastaların 48’inde sağ, 44’ünde sol ve bir hastada bilateral yerleşimliydi. 23 hastada kitle derin lob yerleşimli iken 70 hastada kitle yüzeyel lob ile sınırlıydı. 65 hastaya süperfisial parotidektomi, 19 hastaya total parotidektomi, 8 hastaya total parotidektomi ve boyun diseksiyonu ve iki hastaya radikal parotidektomi uygulandı. Patoloji sonuçları 68’inde benign, 23’ünde malign, iki hastada ise non-neoplastik lezyon olarak raporlandı. Pleomorfik adenoma benign tümörler içerisinde 43 hastada (%51.8) görülürken, Whartin tümörü ikinci sıkılıkta ve 16 hastada (%20.6) görüldü. Dokuz hastada diğer benign neoplazmlar raporlandı. Mukoepidermoid karsinom en sık izlenen malign tümördü. Sekiz hastaya parotidektomi dışında boyun disseksiyonu uygulandı. Hastaların on beşinde komplikasyon gelişti(%16.1). Komplikasyon olarak en sık fasiyal parezi izlendi. Sonuç: Parotis tümörlerinin büyük çoğunluğu benigndir ve tedavisi cerrahidir. Süperfisial parotidektomi en sık uygulanan cerrahi prosedür olup komplikasyon oranı düşüktür. Histopatolojik tanı, kitlenin yerleşimi, tümörün evresi, fasial sinir tutulumu, boyunda palpable lenfadenopati varlığı tedavi yönetiminde oldukça önem arz eder. Malign parotis tümörlerinde süperfisial, total veya radikal parotidektomi uygulanmalı, gerektiğinde boyun disseksiyonu da cerrahi prosedüre eklenmelidir.
Abstract
Objective: We aimed to evaluate clinical presentations, histopathological diagnosis, surgical treatment modalities and complications of patients operated for a parotid gland mass. Methods: Medical records of 93 patients who operated for a parotid gland mass between 2011 and 2019 years evaluated retrospectively. Results: 54 (59.2%) of these patients were male and 39 (40.8%) were female. The mean age of the patients was 44.17±13.20. Tumor was located right sided in 48, left sided in 44 and bilateral in one patient. In 65 patients superficial parotidectomy, in 19 patients total parotidectomy, in 8 total parotidectomy + neck dissection and in two patients radical parotidectomy were used as surgical procedure. Histopathological diagnosis was benign in 68 patients, malign in 23 patients and non-neoplastic in 2 patients. For benign tumors the most common histopathology was pleomorphic adenoma in 43 (51.8%) patients, and the second was Whartin tumor in 16(20.6%) patients. The other benign tumors were seen in 9 patients. mucoepidermoid carcinoma was seen the most common malign tumor. In eight patients with malign tumors neck dissection was performed simultane¬ously with parotidectomy. Complication ratio was 16.1% (15 patient) and facial paresis was the most common complication. Conclusion: The majority of parotid tumors are benign and surgery is the treatment of these. Superficial parotidectomy is the most used surgical procedure and has low complication rate. Histopathological diagnosis, location of the mass, tumor stage, facial nerve invasion and the presence of palpable lymph node on the neck play an important role in the planning of the surgery. Superficial, total or radical parotidectomy should be applied in malignant parotid tumors. In cases of malignant parotid tumors, neck dissection should be added to the treatment.
