ANİİDİYOPATİK SENSÖRİNÖRAL İŞİTME KAYIPLI HASTALARDA MONOSİT / HDL - KOLESTEROL ORANININ DEĞERLENDİRİLMESİ

Özet

Amaç: Monosit / HDL-K oranı (MHO) inflamasyon belirteci olarak kullanılmaktadır. Bu çalışmadaki amacımız ani idiyopatik sensörinöral işitme kaybında (AİSNİK) MHO"yu hastalığın etiyopatogenezi ve prognozu açısından değerlendirmektir. Yöntem ve Gereçler: Kulak Burun Boğaz (KBB) kliniğinde Ocak 2014 - Nisan 2020 tarihleri arasında AİSNİK nedeniyle tedavi edilen hastalar çalışmaya dahil edildi. Kontrol grubu KBB polikliniğine rutin muayene için başvuran hastalardan seçildi. Hastaların tedaviye yanıtları Siegel kriterlerine göre değerlendirilerek iki grup (tam iyileşen, tam iyileşmeyen) oluşturuldu. Grupların MHO’ları analiz edildi. Bulgular: AİSNİK grubunda 70 (tam iyileşen: 31, tam iyileşmeyen:39 ), kontrol grubunda 59 hasta mevcuttu. Gruplar arasında cinsiyet ve yaş açısından anlamlı fark yoktu (p= 0.741, 0.269 sırasıyla). AİSNİK ve kontrol grubunun MHO’ları karşılaştırıldığında istatistiksel olarak anlamlı fark mevcuttu (p<0.001). Prognostik açıdan üç grubun (tam iyileşen, tam iyileşmeyen, kontrol) MHO’ları karşılaştırıldığında da anlamlı fark mevcuttu (p= 0.002).Yapılan ileri analiz sonucu kontrol grubu diğer iki gruptan anlamlı farklıydı (tam iyileşenler, tam iyileşmeyenler). Tam iyileşenler ve tam iyileşmeyenler grupları arasında anlamlı fark yoktu. Sonuç: MHO’nun AİSNİK hastalarında kontrol grubuna göre yüksek çıkması etiyopatogenezde ateroskleroz ve sistemik inflamasyon olduğunu düşündürmektedir. Buna karşın AİSNİK’de MHO’nun prognostik değeri bulunamamıştır.

Abstract

Objective: Monocyte / HDL-C ratio (MHR) is used as an indicator of inflammation. Our aim in this study is to evaluate MHR in terms of etiopathogenesis and prognosis of sudden idiopathic sensorineural hearing loss (SISNHL). Material-Method: Patients treated for SISNHL in the Ear Nose Throat (ENT) clinic between January 2014 and April 2020 were included in the study. The control group was selected from patients who applied to the ENT clinic for routine examination. Patients' responses to treatment were evaluated according to Siegel criteria and two groups (complete recovery, incomplete recovery) were formed. MHRs of the groups were analyzed. Results: There were 70 patients in the SISNHL group (complete recovery: 31, incomplete recovery: 39), 59 patients in the control group. There was no significant difference between the groups in terms of gender and age (p= 0.741, 0.269 respectively). When SISNHL and the control group's MHRs were compared, there was a statistically significant difference (p<0.001). In terms of prognosis, there was also a significant difference when the MHRs of the three groups (complete recovery, incomplete recovery, control) were compared (p= 0.002). As a result of the advanced analysis, the control group was significantly different from the other two groups (complete recovery, incomplete recovery). There was no significant difference between the complete recovery and incomplete recovery groups. Conclusions: The fact that MHR is higher in SISNHL patients compared to the control group suggests atherosclerosis and systemicinflammation in etiopathogenesis. However, the prognostic value of MHR was not found in SISNHL.
