Kolesistektomili Olgularda Displazi ile Eşlik Eden Kanser Birlikteliği

Özet

Amaç: Kolelityazise bağlı gelişen ve kronik inflamasyonla seyreden kronik kolesistitli olgulardaki metaplazi ve displazi gibi karsinogenez basamağında yer alan değişimler, batın içi diğer organ neoplazmlar açısından anlamlı olabilir. Çalışmamızda da olduğu gibi metaplazi ve displazi görülen olgularda kesinlikle batın içi diğer sistemler de taranmalıdır. Yöntem: Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Hastanesi Genel Cerrahi Kliniğinde 01.01.2010 ile 01.07.2012 tarihleri arasında kolesistektomi yapılan nihayi histopatolojik analiz sonucu displazi ile karşılaşılan 68 hasta eşlik eden ve gastrointestinal malignite açısından retrospektif olarak değerlendirildi. Bulgular: Kolesistektomi ameliyatı sonrası histopatolojik inceleme sonuçları displazi/metaplazi ile uyumlu olan 68 hastanın 2’sinde eşlik eden malignite; 2’sinde ise yıllık takipler sonucu ortaya çıkan yeni gastrointestinal sistem maligniteleri belirlenmiştir. Sonuç: Kolesistektomi sonrası patoloji sonuçlarında metaplazi/displazi olan olgular gastrointestinal sistem maligniteleri açısından yakın takibe alınmalıdırlar.

Abstract

Objective: Changes in steps of carcinogenesis such as metaplasia and dysplasia in cases with chronic cholecystitis that develop due to cholelithiasis and progress with chronic inflammation may be significant in terms of other intra-abdominal organ neoplasms. As in our study, cases with metaplasia and dysplasia should be also screened in other intraabdominal systems. Method: Sixty-eight patients who had dysplasia after cholecystectomy operation between 01.01.2010 and 01.07.2012 in General Surgery Clinic of Eskişehir Osmangazi University Faculty of Medicine Hospital were evaluated retrospectively in terms of accompanying gastrointestinal malignancy. Results: In 2 of 68 patients whose histopathological examination results after cholecystectomy were compatible with dysplasia/metaplasia, accompanying malignancy; In 2 of them, new gastrointestinal system malignancies that emerged as a result of annual follow-up were determined. Conclusion: Patients with metaplasia/dysplasia in the pathology results after cholecystectomy should be followed up closely for gastrointestinal system malignancies.
