Tiroid Cerrahisinde İntraopartif Sinir Monitorizasyonu Deneyimimiz

Özet
Rekürren laryngeal sinir yaralanması tiroid cerrahisinden sonra geçici ve kalıcı olarak görülmektedir. Geçici hasar O.4-7.2 ve kalıcı hasar %0-5.2 arasında değişmektedir. Klasik tiroid cerrahisinde diseksiyon ve gözlem sinirİn korunması için kullanılan esas yöntemdir. Son yıllarda sinirin korunmasına yardımcı olması amacıyla intraoperative nöromonitorizasyon (İONM) tekniği geliştirilmiştir. Bu çalışmanın amacı operasyon sırasında nöromonitorizasyon yapmanın rekürren laryngeal sinirin korunmasmdaki etkinliğini araştırmaktır. Materyal ve Metod: Kliniğimizde Eylül 2013--Şubat 2015 tarihleri arasında tiroidektomi uygulanıp İONM yöntemi kullanılan hastalar çalışmaya dahil edildi. Hastaların bilgileri dosyalardan edinilen bilgilerle kayıt altına alındı. Has-- taların yaş, cinsiyet, uygulanan operasyon ve patolojik tanıları değerlendirildi. Tüm hastalara preoperatif dönemde ve postoperatif ilk 15 gün İçerisinde indirek laringoskopi yapılarak kord vokal hareketleri değerlendirildi. Preoperative kord vokal paralizisi olan hastalar çalışma dışı bırakıldı. Bulgular: Çalışma döneminde 140 hastaya tiroidektomi sırasında İONM kullanıldı. Çalışmaya dahil edilen 140 hastanın 110'u kadın (%78.5) ve 30'u erkek olup (%21.5) yaş ortalaması 52.3 (22--85) olarak bulundu. Hastaların 57'si (%40.7) tiroid malignitesi, 47'si (%33.6) nüks noduler guatr veya tamamlama tiroidektomisi, 36'sı (%25.7) multinoduler guatr nedeniyle opere edildiler. Dört hastada (%2.8) operasyon sırasında sinyal kaybı oldu. Bu hastaların 1'i tiroid malignitesi, 3'ü ise nodüler guatr nedeniyle operasyona alındılar. Postoperatif indirek laringoskopide bu has-- taların 'ünde tek taraflı paralizi saptandı (%1.4). Bunların 2'sinde paralizi geçici, 1'inde (% 0.7) kalıcı tek taraflı RLS paralizisi olduğu görüldü. Sonuç: Sonuç olarak İONM tiroid cerrahisinde nervus laringeus inferiorun sağlamlığını operasyon sırasında cerraha doğru bir şekilde göstermektedir. Sinir monitorizasyonu kullanılarak çift taraflı olası sinir hasarını engellemek için evreli tiroidektomi mantıklı bir seçenektir.
Abstract
Recurrent laryngeal nerve injury is seen permanently or transiently after thyroid surgery 0-5.2% 0.4-7.2 %, respectively. Dissection and observation of the nerve is gold standard to prevent nerve injury in thyroid surgery. Recently, intraoperative neuromonitoring (IONM) was discovered to prevent recurrent laryngeal nerve injury in thyroid surgery. The aim of the present study is to discuss our experience on IONM on thyroid surgery. Material and Methods The patients who were performed thyroidectomy with intraoperative neuromonitoring were included in the study. The data of the patients were collected from the files. Patients' age, sex, the kind of operation and pathological diagnosis were evaluated. Indirect laryngoscope was performed to all patients before the surgery. The patients who have laryngeal nerve paralysis preoperatively were excluded from the study. Results IONM was performed on 140 patients during the study period. There were 110 female (78.5%) and 30 male (21.5%) patients in the study group and the overall age was 52.3(22-85) years. The reason of the operation was malignancy for 57 patients (40,7%), recurrent nodular goiter for 47 (33.6%) patients and multiple nodular goiter for 36 (25.7%) patients. The amplitude loosing was occurred in patients during the operation. One of these patients was being operated because of the malignancy and of them had multiple nodular goiter. One side recurrent laryngeal nerve paralysis was identified in patients with postoperative indirect laryngoscope (1.4%). Two of the paralysis were transient and one of them was permanent (0.7%). Conclusion IONM can show to the surgeon that the recurrent laryngeal nerve is undamaged during the operation. Staged thyroidectomy may be logical choice with IONM to prevent bilaterally recurrent laryngeal nerve injury for the patients that suspicious for one side recurrent laryngeal nerve injury during the operation.
