Özet

Amaç: Dilde sukuamoz hücreli karsinom dünyada yaygın olarak görülmektedir. Çalışmamızın amacı erken evre dil karsinomlarında cerrahi tedavi sonuçlarımızı değerlendirmek.Method: 2010 ve 2014 yılları arasında üçüncü basamak sağlık merkezimizde cerrahi ile tedavi edilen 16 T1 dil karsinomlu hastanın kayıtları retrospektif olarak incelendi. Hastaların demografik, histopatolojik ve tedavi özellikleri değerlendirildi. Her bir hastanın T evresi, N evresi, primer tümör invazyon derinliği, cerrahi tedavi, boyun metastazı, TNM evresi, cerrahi tedavi ve adjuvan tedavisi değerlendirildi.Sonuç: Bütün hastalar tümör rezeksiyonu + boyun diseksiyonu ile tedavi edildi. Boyun metastazı 16 hastanın 4 ünde saptandı. Bir hastada cerrahi sınır pozitifliği saptandı. Boyun metastazı olan ve cerrahi sınır pozitifliği saptanan 5 hastaya postoperatif radyoterapi verildi.2 hastada boyun nüksü , 2 hastada lokal nüks tedavisi için cerrahi uygulandı,1 hastamız rekürrens nedeniyle kaybedildi.Özet: Erken evre dil karsinomlarında cerrahi tedavi etkin bir tedavidir. T1 sukuamoz dil karsinomunda yüksek insidansta lenf nodu metastazı eşlik etmektedir. Bu nedenle, bütün T1 evre hastalarda elektif boyun diseksiyonu yapılması göz önünde bulundurulmalıdır

Abstract

Objectives:The squamous cell carcinoma (SCC) of the oral tongue is a common cancer. The aim of this study was to assess the efficacy of primary surgical treatment in the management of early stage oral tongue carcinoma.Method: Sixteen patients treated with primary surgery for T1 oral tongue carcinomas at a tertiary referral center between 2010 and 2014 were retrospectively included in this study. Patients demographic, histopathologic and treatment characteristics were evaluated. T classification, N classification, depth of invasion of the primary tumor, nodal metastasis, overal TNM stage, surgical treatment and adjuvant therapy were assessed for each patient.Results: All patients were treated primary tumor resection combined with neck dissection. Nodal metastases were found in 4 of the 16 patients. One patient had positive surgical margin postoperatively. These 5 patients who had positive surgical margin and/or nodal metastases underwent postoperative radiotherapy. 2 patients had subsequent isolated recurrent disease at the primary site, 2 patients had isolated recurrent disease in the neck. These four patients underwent salvage surgery. One patients with recurrence died of the recurrent disease.Conclusion: Primary surgical treatment is an effective modality in early stage oral tongue carcinoma. T1 sqamous cell carcinoma of tongue is associated with a high incidence of lymph node metastasis. Therefore, elective neck dissection should be considered in all T1 stage patients
