Kronik nörolojik hastalığa bağlı beslenme güçlüğü çeken çocuklarda perkütan endoskopik gastrostomi uygulamaları

Özet
Amaç: Bir çocuk cerrahisi kliniğinin, kronik nörolojik hastalık nedeniyle beslenme güçlüğü çeken çocuklarda, perkütan endoskopik gastrostomi (PEG) uygulamasına ait deneyiminin derlenerek sonuçların yorumlanması; elde edilen sonuçların ışığında Türkiye'de konuyla ilgili varolan durumun değerlendirilmesidir. Gereç ve Yöntem: PEG kateteri ameliyathanede, genel anestezi altında, antibiyotik profilaksisi ile, bükülebilir endoskopi eşliğinde, kılavuz tel üzerinden çekme tekniği ile uygulandı. Evde bakımı sağlayacak kişiye doğru ve uygun kateter bakımı ve kullanımı konusunda eğitim verildi. Bulgular: Sekiz aylık bir süre içinde, ağır beslenme sorunu yaşayan kronik nörolojik hastalıklı 7 çocuğa PEG kateteri uygulandı. Çocukların yaşları 15-120 ay ve ağırlıkları 7,5-16 kg arasındaydı. İşleme bağlı ciddi komplikasyon veya ölüm görülmedi. Hastaların 6'sı yaşamaktadır ve PEG yoluyla beslenmektedir. 80-265 günlük kateter izlem süresinde bütün hastalarda belirgin kilo alımı sağlanmış, hastaneye yatış sıklığı azalmış ve yaşam kalitesi artmıştır. Aileler PEG kateterinin çocuklarının genel sağlığına ve büyümesine katkıları konusunda olumlu düşünmektedir. Sonuç: Elde edilen sonuçlar, beslenme güçlüğü çeken çocuklarda uzun süreli beslenme desteğinin sağlanması için güvenli ve etkili bir yöntem olarak PEG'in tercih edilebileceğini göstermiştir. Bu konuda ülkemiz koşullarına özgü sıkıntıların giderilmesi için çaba göstermeye değecektir.
Abstract
Aim: The aim is to evaluate the results obtained from percutaneous endoscopic gastrostomy (PEG) placement in children with feeding difficulties due to chronic neurological disease in a pediatric surgical clinic and also to evaluate the current situation and circumstances in Türkiye on the subject. Material and Methods: PEG was placed under general anesthesia and antibioticprophylaxis in the operating theatre. Pull technique over a guide wire was used with the aid of fiber optic endoscopy. The caregiver was educated about the proper catheter use and care. Results: During an-8-month period, seven children having severe feeding difficulties and chronic neurological disease underwent PEG placement. They were aged between 15-120 months and weighed between 7,5-16 kg. No procedure-related major complication or mortality occurred. Currently six patients are alive and being fed through the PEG. All gained weight with a decrease in hospital admission rate and an increase in life quality within a catheter follow-up period of 80-265 days. The parents were positive about the effects of PEG on the general well-being and growth of their children. Conclusion: The results obtained showed PEG is safe and effective in providing long-term enteral nutritional support. It definitely deserves a try to overcome the difficulties and prejudice encountered in practice in our country.
