Tip 1 Diyabet Tanısı Almış 05 Yaş Grubu Çocukların Anne Sütü Alımı ve Beslenme ile İlgili Etmenlerin Değerlendirilmesi

Özet
Amaç: Bu araştırma, tip 1 diyabet tanısı almış 0-5 yaş grubu çocuklarda tip 1 diyabet oluşumunu etkileyebilecek beslenme ile ilgili etmenlerin incelenmesi, anne sütü alım durumlarının ve tamamlayıcı besine geçiş dönemlerindeki beslenme özelliklerinin saptanması amacıyla yürütülmüştür. Bireyler ve Yöntem: Araştırmaya, 0-5 yaşları arasında, 36 tip 1 diyabetli çocuk (14 erkek, 22 kız) alınmış ve ebeveynlerine anket formu uygulanmıştır. Ayrıca çocukların bir günlük besin tüketim kayıtları ve antropometrik ölçümleri alınarak beslenme durumları incelenmiştir. Bulgular: Çocukların %55.6sının doğumdan sonra ilk bir saat içerisinde anne sütü aldığı, %8.3ünün 0-3 aylık dönemde anne sütü alımının kesildiği, %68.3ünün ilk 6 ay yalnızca anne sütü aldığı, tamamlayıcı besinlere %11.1inin 0-3 aylık dönemde ve %52.8sinin 7-9 aylarda başladığı belirlenmiştir. 0-6 ayda inek sütüne başlayan çocuk olmadığı ve %55.6sının inek sütüne 10-12 ayda başladığı, tahıllara %41.7sinin 7-9 ayda başladığı ve %66.7sinin hiç D vitamini takviyesi almadığı saptanmıştır. Sonuç: Çocukların diyabet etiyopatogenezinde etkili olabilecek, inek sütü ve tahıllar gibi besinlere erken dönemlerde (<3-4 ay) başlamadığı belirlenmekle birlikte, ilk 6 ay yalnızca anne sütü alımının istenen düzeyde olmaması, <3 ay anne sütü alan çocukların (n=3) olması ve çocukların çoğunun D vitamini desteği almamış olması tip 1 diyabet açısından dikkat edilmesi gereken etmenler olarak görülmektedir. Özellikle tip 1 diyabete genetik yatkınlığı olan çocuklara, β hücre otoimmünitesini ve hastalığın ortaya çıkışını geciktirebilmek ve/veya önleyebilmek için doğumdan itibaren doğru beslenme önerilerinin yapılması önemlidir.
Abstract
Aim: This research was conducted to determine the nutritional factors affecting the risk of type 1 diabetes, breast-feeding and nutritional factors in the complementary feeding period in children with type 1 diabetes aged 0-5 years. Subjects and Methods: In this research, 36 children (14 boys, 22 girls) with type 1 diabetes aged 0-5 years were included and a questionnaire was applied to the parents. Also, nutritional status of subjects was examined with the anthropometric measurements and the dietary intake records. Results: It was found that 55.6% of the children were breastfed in the one hour after birth, 8.3% of the subjects stopped breastfeeding in the 0-3 mo, 68.3% of the children exclusively breastfeed for 6 mo, 11.1% of the children started to complementary foods in the 0-3 mo and 52.8% of the children started to get complementary foods in the 7-9 mo. It was found that none of the children was introduced cows milk in the 0-6 mo, 55.6% of the children were introduced cows milk in the 10-12 mo, 41.7% of the children were introduced cereals in the 7-9 mo and 66.7% of the children do not take vitamin D supplementation. Conclusion: However it was determined that the children were not introduced early (<3-4 mo) with cows milk and cereals, but exclusively breastfeeding rates were inadequate, breastfeeding was stopped in <3 mo (n=3) and no vitamin D supplementation in most of the children are noticing factors for type 1 diabetes. Beginning after delivery, healthy nutritional recommendations for the prevention of the disease and for the prevention of the initiation of the diabetic disease process of type 1 diabetes particularly in genetically susceptible individuals is essential.
