ALT VE ÜST EKSTREMİTE CERRAHİLERİNDE ANESTEZİ DENEYİMLERİMİZ: SON ALTI YILIN RETROSPEKTİF ANALİZİ

Özet
Amaç: Bu çalışmanın amacı, son 6 yılda (2008-2013) kliniğimizde yapılan alt ve üst ekstremite ameliyatlarındaki anestezi yöntemlerinin değerlendirilmesidir. Yöntem: Bu çalışmada, Atatürk Üniversitesi Tıp Fakültesi Hastanesi Ameliyathanesi’nde 2008-2013 yıllarında gerçekleştirilen alt ve üst ekstremite ameliyatlarının hastane bilgisayar sistemindeki kayıtları incelendi. Operasyonlarda uygulanan anestezi yöntemlerinin yıllara göre dağılımı analiz edildi. Bulgular: Kliniğimizde 2008-2013 yıllarında, toplam 10636 alt ve üst ekstremite cerrahisi gerçekleştirildi. Hastaların %54,5’inde genel anestezi, %45,3’ünde bölgesel anestezi tekniklerinin uygulandığı bulundu. Bölgesel anestezi uygulama oranı 2008 yılında %44,3 iken 2013 yılında bu oranın %49’a yükseldiği, ancak bu artışın istatistiksel olarak anlamlı olmadığı görüldü. En sık kullanılan bölgesel anestezi yöntemlerinin, 2008 yılında spinal anestezi (%33,5), 2013 yılında periferik sinir blokları (%40,8) olduğu bulundu. Son 3 yılda periferik sinir bloğu uygulamalarında belirgin artış olduğu, başarısızlık oranlarında ise azalma olduğu gözlendi. 2013 yılında en fazla uygulanan periferik sinir bloğu yönteminin femoral + siyatik + psoas blok olduğu bulundu. Sonuç: Kliniğimizdeki alt ve üst ekstremite ameliyatlarında 2008-2013 yıllarında en yaygın kullanılan anestezi yöntemi genel anestezidir. Kliniğimizde en yaygın kullanılan bölgesel anestezi yöntemi periferik sinir bloklarıdır. Son 3 yılda periferik sinir blokları tekniğinin kullanımında önemli bir artış vardır.
Abstract
Objective: The purpose of this study was to evaluate the anesthesia methods which was employed for lower and upper extremity surgeries in our clinic in last six years (2008-2013). Method: In this study; the records that are stored in the hospital computer system of lower and upper extremity surgeries performed between the years of 2008-2013 at Ataturk University Medical Faculty were analyzed. Annual distribution of anesthesia methods in operations were studied. Results: In our clinic, a total number of 10636 lower and upper extremity surgeries have been performed between the years 2008-2013. Rate application of general anaesthesia was 54.5% and it was 45.3% for regional anesthesia. While regional anesthesia application rate was 44,3% in 2008, this ratio increased to 49% in 2013, but this increase was not statistically significant. The most commonly used regional anesthesia method was spinal anesthesia (33.5%) in 2003, and peripheral nerve blocks (40.8%) in 2013. In last 3 years, a significant increase in peripheral nerve block applications and a decrease in the failure rate of it were observed. The most widely applied method in peripheral nerve blocks in 2013 was found to be the femoral+psoas+sciatic nerve block. Conclusion: The most commonly used anesthesia technique for lower and upper extremity surgeries was general anesthesia in our clinic in years 2008-2013. The most widely used regional anesthetic method in our clinic was peripheral nerve block. There has been a significant increase in the use of peripheral nerve blocks technique in the last three years.
