Neovasküler Glokomlu Hastalarda Uygulanan Mitomisin-C’li Trabekülektomi ve Ahmed Glokom Valv İmplantasyonunun Prognoz ve Sonuçları

The Results and Prognosis of Ahmed Glaucoma Valve Implantation and Trabeculectomy with Mitomycin C in Patients with Neovascular Glaucoma

Özet

Amaç: Neovasküler glokomlu (NVG) olgularda trabekülektominin ve Ahmed Glokom Valv (AGV) implantasyonunun sonuçlarının ve prognozunun değerlendirilmesi. Gereç ve Yöntemler: Nisan 2012-Mayıs 2017 tarihleri arasında mitomisin C’li (MMC) trabekülektomi ve AGV implantasyonu uygulanan NVG’lu hastaların dosyaları retrospektif olarak incelendi. Başvuru anında ve kontrollerde ayrıntılı oftalmolojik muayene yapıldı. Yaş, cinsiyet, takip süresi, en iyi düzeltilmiş görme keskinliği (EDGK), göz içi basıncı (GİB), komplikasyonlar kaydedildi. GİB’in 5-21mmHg arasında olması cerrahi başarı kabul edildi. İstatistiksel analizlerde paired-t testi kullanıldı, p<0.05 olması anlamlı kabul edildi. Bulgular: NVG’lu 40 hastanın 20’sine MMC’li trabekülektomi, 20’sine AGV implantasyonu uygulandı. Trabekülektomi grubundakilerin 12’si (%60) erkek, 8’i (%40) kadın, yaşları 54.3±15.56; AGV grubundakilerin ise 11’i (%55) erkek, 9’u (%45) kadın, yaşları 53.1±18.59 idi. Takip süresi trabekülektomi grubunda 14.3, AGV grubunda 15.1 ay idi. Başvuru anında ve 12.ayda EDGK’de gruplarda istatistiksel olarak anlamlı fark görülmedi (p=0.41). Tedavi öncesi GİB trabekülektomi grubunda 42.35±8.66 mmHg, AGV grubunda 40.61±7.85 mmHg; 12.ayda sırayla 14.99±4.55 mmHg, 15.09±4.38 mmHg idi (p=0.53). Cerrahi başarı oranı trabekülektomi grubunda %73, AGV grubunda %65 idi (p=0.32). Trabekülektomi grubundaki hastaların 4’ünde (%20) sığ ön kamara, 4’ünde (%20) hifema, 3’ünde (%15) hipotoni, 3’ünde (%15) katarakt, 2’sinde (%10) vitre içi hemoraji (VİH), 1’inde (%5) koroid dekolmanı, 1’inde (%5) retina dekolmanı; 1’inde (%5) blebite bağlı endoftalmi görüldü. AGV grubundaki hastaların ise 4’ünde (%20) enkapsüle bleb, 3’ünde (%15) sığ ön kamara, 2’sinde (%10) hipotoni, 2’sinde (%10) hifema, 1’inde (%5) kornea ödemi görüldü. Sonuç: NVG’lu hastalarda MMC’li trabekülektomi ve AGV implantasyonu cerrahi başarı açısından karşılaştırıldığında istatistiksel olarak anlamlı bir fark görülmemiştir. Sonuç olarak her iki cerrahi de primer prosedür olarak tercih edilebilir.

Abstract

Purpose: Evaluation of the outcome and prognosis of trabeculectomy and Ahmed Glaucoma Valv (AGV) implantation in patients with neovascular glaucoma (NVG). Materials and Methods: The fi les of patients with NVG who underwent trabeculectomy with mitomycin C and AGV implantation between April 2012 - May 2017 were retrospectively analyzed. Detailed ophthalmologic examination was performed at the time of admission and at the controls. Age, sex, follow-up period, best corrected visual acuity (BCVA), intraocular pressure (IOP), complications were recorded. IOP between 5 and 21 mmHg was accepted as surgical success. Paired-t test was used for statistical analysis, p<0.05 was considered signifi cant. Results: Twenty of 40 patients with NVG underwent trabeculectomy and 20 of them had AGV implantation. In the trabeculectomy group, 12 (60%) were male and 8 (40%) were female, age 54.3 ± 15.56; In the AGV group, 11 (55%) were male and 9 (45%) were female and their age was 53.1 ± 18.59. The follow-up period was 14.3 months in the trabeculectomy group and 15.1 months in the AGV group. There wasn’t statistically signifi cant difference in visual acuity between groups at the time of admission and at the 12th month (p=0.41). IOP was 42.35±8.66 mmHg in the trabeculectomy group and 40.61±7.85 mmHg in the AGV group before the treatment; 14.99±4.55 mmHg and 15.09±4.38 mmHg respectively in the 12th month (p=0.53). Surgical success rate was 73% in trabeculectomy group, 65% in AGV group (p=0.32). There were shallow anterior chamber in 4 (20%) patients in the trabeculectomy group, 4(20%) had hyphema, 3(15%) had hypotonia, 3(15%) had cataract, 2(10%) had vitreous hemorrhage, 1(5%) had choroidal detachment, 1(5%) had retinal detachment, 1(5%) had blebitis-induced endophthalmitis. 4(20%) patients in the AGV group had encapsulated bleb, 3(15%) had shallow anterior chamber, 2(10%) had hypotonia, 2(10%) had hyphema, 1(5%) had corneal oedema. Conclusion: There was no statistically signifi cant difference between trabeculectomy with MMC and AGV implantation in patients with NVG. As a result, both surgeries can be preferred as a primary procedure.
