Özet
Giriş: Stileler sivri ya da sert metal uçlara bağlı travmayı önlemek ve entübasyon sonrasında çıkarılmalarını kolaylaştırmakamacıyla plastik kaplıdırlar. Bununla birlikte, plastik kaplamalar problemlere yol açabilir. Bu olgu sunumunda, dört numaralıspiralli bir endotrakeal tüpten çıkarılması sırasında stilenin plastik kaplamasının sıyrıldığı bir deneyimi sunmayı amaçladık.Olgu Sunumu: Genel anestezi altında adenoidektomi planlanan 3 yaşında çocuk, plastik kaplı bir stile yardımıyla 4 numaralıspiralli endotrakeal tüp kullanılarak entübe edildi. Çıkarılması sırasında stilenin distal kısmındaki plastik kaplamanın yerindeolmadığı ve endotrakeal tüp lümeninin sıyrılıp kopan parça ile tıkandığı fark edildi. Hemen endotrakeal tüp çıkarıldı ve stileüzerindeki sıyrılarak kopan plastik kaplamanın tek parça olduğu doğrulandı. Hasta başka bir endotrakeal tüp ve stile ile tekrarentübe edildi. Devamında cerrahi işlem, idame ve derlenme periyodları sorunsuzdu. Sonuç: Bu olgu sunumu, stilenin plastik kaplamasının sıyrılmasının nadir fakat potansiyel olarak ciddi bir komplikasyonolduğunu vurgulamaktadır. Ayrıca, bu komplikasyonu önleme, erken tanı ve oluştuğunda havayolu yönetimi ile ilişkili tavsiyelersunulmuştur
Abstract
Introduction: Stylets are plastic coated to prevent injury from sharp or rough metal ends and to facilitate their removal afterintubation. However, the plastic coating may lead to problems. In this case report, we aimed to report an experience in which theplastic coating of the stylet sheared during its removal from a 4 mm reinforced endotracheal tube.Case Report: A 3-year-old child scheduled for adenoidectomy under general anesthesia was intubated using a 4 mm reinforcedendotracheal tube with the aid of a plastic coated stylet. At removal, it was noticed that the plastic coating over the distal part ofthe stylet was missing and the lumen of the endotracheal tube was obstructed by the distal sheared-off portion. The endotrachealtube was immediately removed and confirmed that the sheared-off part is a single piece. The patient was re-intubated with anotherendotracheal tube and stylet. Subsequent surgical procedure, the maintenance and the recovery periods were uneventful.Conclusion: This case report highlights that a shearing off the plastic coating of the stylet is a rare, but potentially seriouscomplication. Furthermore, suggestions for prevention, early diagnosis and airway management when plastic coating has beensheared-off were presented
