Özet

Amaç: Sepsis, infeksiyona yanıt olarak organ hasarına ilerleyen ciddi bir tıbbi durumdur. Dünyada önemli bir ölüm nedeni olarak, yoğun bakımda araştırma ve tartışma konusu olmaya devam etmektedir. Tedavi hızlı tanıma, resüsitasyon, hemodinamik destek, infeksiyon kaynağının kontrolünü ve erken antibiyotik tedavisini içerir. Hemodinamik destek; güçlü kanıtlarla noradrenalin ilk seçenektir. Olgu sunumu: Septik şok nedeniyle takip edilen 2 olgumuzda doku perfüzyonunu sağlamak amacıyla yüksek doz vazopressör ilaç kullanılıyordu. Metilen mavisi ile muamele, yüksek ilaç dozlarının azaltılmasında yararlı olmuştur. Sonuç: Metilen mavisi birçok şok türünde kullanılır. Yüksek doz noradrenalin gereksinimi olan septik şoktaki hastalarda metilen mavisi kullanımının doz azaltılmasını sağlayarak yüksek doza bağlı yan etkilerin önlenmesinde yararlı olabileceğini inanıyoruz.

Abstract

Objective: Sepsis is a serious medical condition that progresses to organ damage in response to infection. As a major cause of death in the world, it continues to be the subject of research and discussion in intensive care units. Treatment includes rapid recognition, resuscitation, hemodynamic support, control of the source of infection and early antibiotic therapy. In hemodynamic support noradrenalin is the first alternative with strong evidence. Case presentation: In two patients followed up with septic shock; high-dose vasopressor drugs were being used to achieve tissue perfusion. Treatment with methylene blue has become beneficial in reducing high doses of drugs. Conclusion: Methylene blue is used in many types of shock, We believe that the use of methylene blue in patients with septic shock requiring high-dose noradrenaline may be beneficial in preventing dose-related side effects by reducing the dose.
