ORTOPEDİ VE TRAVMATOLOJİ CERRAHİSİNDE ANESTEZİ YÖNTEMLERİNİN RETROSPEKTİF ANALİZİ

Özet
Amaç: Bu çalıflmada ortopedik ve travmatoloj ik cerrahi geçiren vakalarda uygulanan anestezi yöntemlerini ve bu yöntemlerin perioperatif mortalite ve morbiditeye etkisini retrospektif olarak arafltırmayı amaçladık. Yöntem: Bu çalıflmada 2006-2012 tarihleri arasında alınan ortopedi ve travmatoloj ik cerrahi geçiren vakaların yafl, cinsiyet, tanı, uygulanan anestezi yöntemi, ASA skoru, kan transf üzyonu yapılıp yapılmadığı, ameliyatın acil veya elektif olma durumu, ek hastalık varlığı, perioperatif görülen komplikasyonlar ve hastanın ameliyat sonrası yoğun bakım gereksinimi incelendi. Bulgular: Kliniğimizde 2006-2012 yılları arasında 2814 olgu ortopedik cerrahi geçirdi. Bu olguların %62,1i erkek, %37,9u kadın idi. Olguların %68,5i genel, %31,5si rejyonel anestezi ile opere oldu. Ameliyat sonrası olguların %97,4ü postoperatif servise, %2,6sı postoperatif yoğun bakım ünitesine (YBÜ) gönderildi. YBÜ'ye alınan olguların %23,6sı ASA I, %14ü ASA II, %62,5i ASA III idi. Tüm olguların %10unda en az bir komplikasyon görüldü. Genel anestezide komplikasyon görülme oranı %10,3, rejyonel anestezide %9,1 olarak tespit edildi. Tüm olguların %12,4üne kan tranf üzyonu yapıldı. Olguların 182sinde ek hastalık mevcut iken, 2632sinde ek hastalık yoktu. Ek hastalık bulunan olguların %69u ameliyat sonrası servise, %31i YBÜye gönderildi. Operasyon süresince en f azla görülen komplikasyon hipotansiyon, ikinci sırada bradikardi oldu. Sonuç: Ortopedi ve travmatoloj i cerrahisinde rejyonal anestezi yöntemleri perioperatif komplikasyonları azaltabilir ancak olgunun karakteri seçilecek anestezi tekniğinde esas belirleyici olmalıdır. Yüksek riskli veya yandafl hastalığı bulunan hastaların acil ve kompleks cerrahilerinde kan transf üzyonu gereksinimi göz önüne alınarak anestezi planı yönetilmeli ve postoperatif yoğun bakım takibi muhakkak değerlendirilmelidir.
Abstract
Obj ective: In this study, the cases which were operated in our Orthopaedics and Traumatology clinic were retrospectively analyzed. However, we aimed to investigate retrospectively the eff ect of the anesthesia methods on perioperative mortality and morbidity. Method: In this study, age, gender, diagnosis, type of anesthesia, ASA clinical classif ication, intraoperative blood tranf usion requirement, type of surgery, emergency or elective, the presence of comorbid diseases, perioperative complications, postoperative transf er of the patients to the intensive care unit (ICU) requirement was analyzed in cases of orthopedics and traumatology surgery, between the dates of 2006-2012. Results: Between the dates of 2006-2012, 2814 patients had Orthopaedics and Traumatology surgery. Sixty two point one percent of these patients were male and 37,9% were f emale, 68,5% of the cases were operated with general anesthesia, 31,5% of the cases with regional anesthesia. Af ter the surgery 97,4% of the patients were transf erred to service, 2,6% were transf erred to intensive care unit. The patients who received intensive care were 23,6% of ASA I, 14% ASA II, 62,5% ASA III. In 10% of all cases at least one complication was observed. The incidence of complications is, 10,3% in general anesthesia, 9,1% in regional anesthesia. In 12,4% of all patients blood transf usion was perf ormed. Of the all cases, 182 patients had additional diseases, whereas 2632 patients had no additional disease. Postoperatively, 69% of patients with comorbid disease was transf erred to service and 31% of them were transf erred to the ICU. The most common complication during the operation was hypotension, latter was bradycardia. Conclusion: In orthopaedic and traumatology surgery, regional anesthesia techiques may decrease the risk of perioperative complications but the medical risk f actors of patients must be the basic predictor f or the decision making of the anesthesia techniques. In the complex surgery of the patients with high ASA class or with accompanying diseases, anesthesia decisions should be made keeping in mind that there may be a need f or blood transf usion. Postoperative f ollow up of the intensive care should def initely be evaluated as well.
