Özet
Geriatrik hastalar fizyolojik, farmakolojik, psikolojik ve sosyal yönden genç hastalardan farklı özelliklere sahiptir. Bu hastalarda anesteziye bağlı komplikasyonların genç nüfusa oranla yüksek olduğu bilinen bir gerçektir. Bu nedenle özellikle ileri yaşlı (80 yaş üstü) hastalarda uygulanacak anestezi yöntemi çok daha önem kazanmaktadır. Erken postoperati f dönemde görülen kogniti f bozukluklar (deliryum, konfüzyon vs.) bu hastalarda önemli komplikasyonlar olarak karşımıza çıkabilmektedir. Genel anestezinin yaşlı hastalarda erken postoperati f dönemde kogniti f bozukluklara yol açabileceği ve bölgesel anestezinin genel anesteziye kıyasla daha az komplikasyonlara sebep olduğu ile ilgili çok sayıda yayın bulunmaktadır. Yazımızda, idrar yapamama, dizüri, idrar kaçırma şikayetleri ile başvuran ve 75 gün arayla iki kez spinal anestezi altında başarılı bir şekilde TUR-P operasyonu gerçekleştirilen 102 yaşında bir erkek hastayı sunduk. Hastanın özgeçmişinde diabetes mellitus tip-II, hipertansiyon ve prostat malignitesi tanıları mevcuttu. Hastanın preoperati f laboratuvar değerleri normal, vücut kitle indeksi 23.4 ve anestezi riski ASA III olarak değerlendirildi. Sonuç olarak ileri yaşlı hastalarda her hastayı ayrı ayrı değerlendirerek, genel anestezinin olumsuz etkilerine kıyasla spinal anestezinin iyi bir hazırlık ve dikkatli bir yönetim ile daha avantajlı bir şekilde uygulanabileceğini düşünüyoruz.
Abstract
Elderly patients di ffer from younger patients in terms of physiological, pharmacological, psychological and social characteristics. It is known that complications related to anesthesia in the geriatric patients are higher compared to the young people. In particular, anesthesia applied to the patients with advanced age (over 80 years) is highly important. Cognitive deterioration (delirium, confusion, etc.) is an important problem in early postoperative period in the elderly patients. It has been reported that general anesthesia in elderly patients may lead to cognitive disorder in early postoperative period while regional anesthesia leads to fewer complications compared to general anesthesia. A transurethral resection of the prostate was carried out under spinal anesthesia twice with 75 days interval successfully in a 102-year-old patient with inability to urinate, dysuria and urinary incontinence due to prostate malignancy. He was previously diagnosed with diabetes mellitus type II and hypertension. His preoperative laboratory values were in normal range, body mass index was 23.4 and American Society of Anesthesiologists physical status (ASA) was ASA-III. In conclusion, we believe that after evaluating each elderly patient individually in terms of negative effects of anesthesia, spinal anesthesia with a good preparation and careful management may be more favorable compared to general anesthesia
