Total Kalça Protezi Cerrahisi Sırasında Periprostetik Femur Kırığı Geçiren Hastaların Radyolojik ve Fonksiyonel Sonuçlarının Değerlendirilmesi

Özet

Amaç: Total kalça protezi son 30 yıl içinde fonksiyonel ve rad- yolojik olarak başarılı sonuçları bildirilen ortopedik cerrahilerin- dendir. Bu cerrahi sırasın da meydana gelen periprostetik femur kırıklarının ilerleyen dönemde hastada yarattığı fonksiyonel so- nuçları incelemek çalışmamızın ana konusuydu. Yöntemler: 2005-2011 yılları arasında merkezimizde tedavi edilmiş ve periprostetik femur kırığı geçirmiş olan 91 hastanın 94 kalçası çalışmaya dahil edildi. Oluşan kırık tipleri Vancouver sınıflamasına göre sınıflandırıldı. Kırık tedavisinde kullanılan yöntemler kaydedildi ve yine açılım şekilleri, kırığın ameliyatın hangi basamağında (raspalama, oyma, stem çakma, redüksiyon sırasında) oluştuğu not edildi. Hastalar, kontrole çağrılarak pelvis AP ve opere olan taraf femur AP grafileri çekildi. Yine aynı za- manda son muayenelerin de WOMAC skorlaması yapıldı. Daha önceki kontrollerinde çekilmiş olan grafileri ile son kontrolle- rinde ki grafileri arasında femoral stemde çökme olup olmadığı incelendi. Bulgular: Kırıkların çoğu femoral stemin çakılma aşamasında oluştuğu gözlemlendi (Hastaların %81’i). Hastaların son kontrol- lerinde ki WOMAC skoru ortalama 27 (8-81) olarak belirlendi. Kalçalardan 12 sinde (%13) ilerleyen zamanlar da protezde çökme tespit edilmiş ve bu kalçalardan 6’sı revizyon cerrahisi geçirmişti. Çökme görülen hastaların kırık tipleri ile olan ilişkilerine bakıldı- ğın da istatistiksel olarak anlamlı fark bulunamadı (p=0,2). Yine çökme görülen hastaların ortalama WOMAC skorları 57’ye yük- seldi ve istatistiksel olarak anlamlı bulundu (p<0,0001). Sonuç: TKP sırasında oluşan intraoperatif femur kırıkları, has- talarda uzun dönemde femoral stemde çökmeye yol açarak imp- lant yetmezliğine yol açabilmektedir. Bu çökmenin oluşan kırık tipi ilişkisi çalışmamızda ortaya konamamıştır. Çökme nedeniyle hastalarda fonksiyonel açıdan anlamlı olarak bozulma görülmek- tedir.

Abstract

Objective: Total hip arthroplasty is a functional and radiologi- cally successful orthopedic operation, commonly performed over the last 30 years. The aim of our study was to investigate the prevalence and functional results of periprosthetic femur frac- tures occurring during the course of surgery. Methods: Institutional review board approval was obtained prior to initiation of this study. We examined 94 hips of 91 patients, who underwent surgery in our centre between 2005 and 2011 and had a periprosthetic femur fracture. The fractures were clas- sified based on the Vancouver classification. The methods used for fracture treatment were recorded, and during the surgery the step at which the fracture occurred was noted (broaching, ream- ing, stem insertion or reduction). At final follow-up, pelvis and femur anterior-to-posterior radiographies of the operated side were taken, and WOMAC scores were assessed. We compared the radiographs taken at the previous controls and last controls, to determine if there was a subsidence of the femoral stem. Results: Most of the fractures were occurred during the insertion of femoral stem phase (81% of patients). The mean WOMAC score at the last follow-up of the patients was 27 (8-81). Twelve (13%) hips had subsidence in the prosthesis, of which six un- derwent revision surgery. There was no significant difference be- tween patients with or without subsidence and the fracture type (p=0.2). The mean WOMAC score of patients with femoral sub- sidence increased to 57, which was significantly different from that of patients without femoral subsidence (p<0.0001). Conclusion: Intraoperative femur fractures occurring during total hip arthroplasty may lead to long-term subsidence of the femoral stem and subsequent implant failure. We could not de- termine the relationship between this type of fracture and subsid- ence. There was no significant deterioration in the hip function due to subsidence.
