Özet
Obturator herni, tüm abdominal hernilerin %0,07-%1'inioluşturan abdominal hernilere bağlı ince bağırsaktıkanıklıklarının da %0,2- %1,6'sına sebep olan nadir birdurumdur. Çok nadir olmalarına rağmen abdominalherniler içerisinde %13 ile %40 arasındaki oranlarla enyüksek mortaliteye sahiptirler. Obturator hernilerin tanıkonulması zordur. Genellikle cerrahi tedavi, klinik veradyolojik tanı koymadaki zorluktan dolayı gecikir. Bizburada 83 yaşında, uzamış 10-12 günlük aralıklıbağırsak tıkanması atakları şikayeti ile Acil Servis'ebaşvuran bir kadın hastayı sunduk. Bilgisayarlıtomografi (BT) çekildi ve terminal ileum düzeyinde sağfemoral kanal ve obturator kanal seviyesinde terminalileuma yakın düzeyde tıkanıklık görülmesi üzerine hastaameliyata alındı. Sağ obturator kanalda inkarsere olanterminal ileum segmenti ve bu segment lateralindekiperforasyon alanı saptandı. Rezeksiyon, anastomoz veobturator herni onarımı yapıldı. Yaşlı, kilo kaybı vekronik hastalıkları olan kadın hastalarda tekrarlayanileus atakları olması durumunda obturator herni şüphesiuyanmalıdır. İyi sonuçlar için erken tanı önemlidir
Abstract
An obturator hernia is a rare condition accounting for0.07-1% of the abdominal hernias; causing 0.2-1.6%of the small bowel obstructions due to the hernias.Even if they are really rare, they have the highestmortality rate of all the abdominal wall herniasreported to be between 13% and 40%. Obturatorhernias are difficult cases to make a diagnosis.Surgical intervention is often delayed because ofclinical and radiological diagnostic difficulty in thediagnosis. We present the case of an 83-year-old ladywho presented with prolonged, over 10-12 days ofintermittent small bowel obstruction admitted to theEmergency Room. Computerised tomography (CT)was taken. The cut off sign at terminal ileum justclose to the right femoral canal & obturator canal wasseen and the patient was taken to the operation room.The incarcerated loop in the right obturator canal anda perforation at the side of the terminal ileum wasseen in the surgery. Resection and anastomosis withthe repair of the hernia was done. A high suspicion forobturator hernia should be maintained in the elderlyfemale emaciated patients with chronic diseasepresenting with recurrent episodes of ileus.
