Özet

Melazma özellikle Asyalılarda ve Hispaniklerde görülen ,yaygın edinsel pigmenter bir hastalıktır.Yüzün güneşe maruz kalan ya- naklar, alın, çene ve burun gibi bölgelerinde gri-kahverengi ya- malarla karakterizedir. Hastalar üzerinde psikolojik bir etki ya- ratır ve yaşam kalitesini düşürebilir. Hem doktor hem de hasta açısından tedavisi uzun süredir hayal kırıklığına neden olan bir durumdur. Aralıksız sıkı bir güneşten korunma ve diğer tetikle- yicilerden kaçınma esastır. Hidrokinon(HK) topikal monotera- pide en sık kullanılan ajandır. Topikal üçlü kombinasyon formül tedavide halen altın standarttır. Kimyasal peeling ve lazerlerin postinflamatuvar pigmentasyona yol açma riskleri yüksektir. Oral traneksamik asit (TA )konvansiyonel tedavilere yanıt ver- meyen veya kısmi yanıt veren olgularda düşünülebilir, ancak bu ajan için etkinlik ve güvenirlilik çalışmalarına gerek vardır. Me- lazma patogenezinde dermal değişikliklerin ve vaskülarizasyonun öneminin anlaşılması yeni tedavi seçeneklerinin geliştirilmesini sağlayacaktır.

Abstract

Melasma is a common acquired pigmentary disorder and is par- ticularly seen among Asians and Hispanics. It is characterized by greyish-brown patches in areas of the face exposed to the sun, such as the forehead, cheeks, lips, and nose. Melasma has a psy- chological effect on affected patients and can reduce their quality of life. Since long, it has been a frustrating condition in terms of treatment for both dermatologists and patients. Continuous strict sun protection and avoding other triggers are essential. Hy- droquinone is the most commonly used agent in topical mono- therapy. A topical triple-combination formula is still the gold standard for treating melasma. Chemical peels and lasers lead to a high risk of post-inflammatory pigmentation. Oral tranexamic acid may be considered for those who fail or who only partially respond to conventional therapy; however, further studies are required to fully elucidate the efficacy and safety of this agent. Understanding dermal changes and the importance of vascular- ization in the pathogenesis of melasma will result in the develop- ment of new treatment options.
