PRESENCE, LOCATION AND COURSE OF MANDIBULAR INCISIVE CANAL AND INTER-EXAMINER VARIATION: A SPIRAL CT SCAN STUDY

Özet
Amaç: Bu çalışmanın amacı insisiv kanalın mevcudiyeti/ lokalizasyonu, rotası ve boyutlarını spiral bilgisayarlı tomografide (BT) değerlendirmek ve insisiv kanal ile ilgili ölçümlerde gözlemciler arası potansiyel farkları analiz etmektir. Materyal ve Metod: Toplam 90 spiral BT taramasının, önceden bir BT uzmanı tarafından belirlenmiş kesitleri, üç ayrı gözlemci tarafından (iki oral radyolog, bir periodontolog) analiz edildi. İnsisiv kanalın vertikal ve horizontal çapı, bukkal ve lingual sınırlara horizontal mesafesi, diş köklerine /alveoler sırta ve inferior sınıra olan vertikal mesafesi BT kesitlerinde ölçüldü. Bulgular: Üç gözlemci arasında insisiv kanal ile ilgili ölçümlerde iyi derecede uyum gözlendi. İnsisiv kanal değişken mesafe ve çaplarda olabilir. Kanal medyale ilerlediğinde vertikal ve bukkolingual çapında azalma trendi dikkat çekti. İnsisiv kanal tüm bölgelerde bukkalde konumlanmıştı ve orta hatta doğru apikale eğimliydi. Sonuç: Potansiyel nörovasküler zarar riski olduğunda cerrahi uygulanacak alanın komplikasyonlardan korunmak için BT ile incelenmesi gereklidir.
Abstract
Background and Aim: The aim of the present study was to assess the spiral computed tomography (CT) scans for the presence/ location, course and dimension of the incisive canal and also to analyze the potential inter-examiner differences regarding incisive canal related-measurements. Materials and Methods: A total of 90 spiral CT scans were analyzed by three different observers (two oral radiologist and one periodontist) on predetermined sections by a CT specialist. Vertical and horizontal diameter of the incisive canal, the horizontal distance to the buccal and lingual borders, vertical distance to the teeth apicies/crest of ridge and inferior border were measured on CT scan sections. Results: There was a good level of agreement regarding incisive canal related measurements among the three observers. Incisive canal may be located in a large variety of distances and diameters. A trend of decrease in the vertical and buccolingual diameter was noted as the canal proceeded medially. Incisive canal was more buccally oriented in all regions and sloped apically towards midline. Conclusion: When there is a high risk for potential neurovascular damage, CT scan of the surgical site is essential to avoid any complications.
