EVALUATION OF POSTOPERATIVE PAIN AFTER SINGLE-VISIT ROOT CANAL TREATMENT USING THREE DIFFERENT INSTRUMENTATION TECHNIQUES: A RANDOMISED CLINICAL TRIAL

Özet

Background and Aim: To evaluate postoperative pain after single-visit root canal treatment using three different instrumentation techniques. Materials and Methods: Fifty-seven permanent maxillary premolar teeth with vital pulps (57 patients) requiring endodontic treatment were included in this study. The teeth in group 1 (n= 19) were instrumented with Protaper Universal to F2 (25/.08) by continuous rotation. The teeth in group 2 (n= 19) were instrumented with OneShape (25/.06) by continuous rotation. The teeth in group 3 (n= 19) were instrumented with WaveOne Primary (25/.08) by reciprocating movement. After chemomechanical preparation, root canals were obturated at the same visit. Postoperative pain evaluation forms were given to the patients, who were asked to record pain levels for seven days after obturation. The Kruskal-Wallis test was used for statistical analysis; differences were considered statistically significant at p< .05. Results: Statistically significant difference was found between the Wave One Primary and Protaper Universal groups immediately after the disappearance of local anaesthesia and at 12 hours, with the WaveOne Primary group reporting significantly more postoperative pain. The WaveOne Primary technique was also associated with significantly more postoperative pain than the other two groups at 1, 2, and 3 days. There were no statistically significant differences between the groups at 4, 5, 6, and 7 days. Conclusion: The continuous rotation technique resulted in significantly less postoperative pain than the reciprocation technique.

Abstract

Amaç: Bu çalışmanın amacı üç farklı enstrümantasyon tekniği kullanılarak yapılan tek seans kanal tedavisi sonrası postoperatif ağrının değerlendirilmesidir. Gereç ve Yöntemler: Endodontik tedavi gerektiren, vital pulpalı 57 daimi üst küçük azı dişi (57 hasta) çalışmaya dahil edildi. Grup 1’deki (n=19) dişler, Protaper Universal F2 (25/.08)’ ye kadar sürekli rotasyonla prepare edildi. Grup 2’deki (n=19) dişler, OneShape (25/.06) ile sürekli rotasyonla prepare edildi. Grup 3’teki (n=19) dişler, resiprokasyon hareketi ile Wave One Primary (25/.08) kullanılarak prepare edildi. Kemomekanik preparasyondan sonra kök kanallar aynı seansta obture edildi. Hastalara postoperatif ağrı değerlendirme formu verildi ve obturasyondan sonraki yedi gün boyunca ağrı düzeyini işaretlemeleri istendi. İstatistiksel analiz için Kruskal Wallis testi kullanıldı. Farklılıklar, p<.05 ise istatistiksel olarak anlamlı kabul edildi. Bulgular: Lokal anestezinin sonlanmasından hemen sonra Protaper Universal ve WaveOne grupları arasında istatistiksel olarak anlamlı farklılık bulundu ve 12. saatte, WaveOne grubu Protaper Universal grubuna göre anlamlı derecede daha fazla postoperatif ağrı sergiledi. 1., 2., 3. günlerde, WaveOne grubu diğer iki gruba göre anlamlı derecede daha fazla postoperatif ağrı ile sonuçlandı. Gruplar arasında 4., 5., 6. ve 7. günlerde istatistiksel olarak anlamlı fark yoktu. Sonuç: Sürekli rotasyon tekniği, resiprokasyon tekniğine göre tedavi sonrası ağrının belirgin olarak daha az olmasına neden olmuştur.
