Özet

Giriş ve Amaç: Total larenjektomi geçirmiş olan hastalarda solunum, konuşma, yutma ve koku alma gibi temel fonksiyonların etkilenmesi nedeniyle yaşam kalitesi bozulmaktadır. Yaşam kalitesi sorgulandığın-da, hastaların konstipasyon veya dışkılama özellikleri sıklıkla göz ardı edilmektedir. Bu çalışmanın amacı, ileri evre larenks kanseri tanısıyla farklı tedavi stratejileri uygulanmış hastalarda, standardize sorgulama-larla, fonksiyonel konstipasyon sıklığını değerlendirmek ve konstipasyo-nun yaşam kalitesine etkisini ortaya koymaktır. Gereç ve Yöntem: İleri evre larenks kanseri tanısıyla total larenjektomi uygulanmış 15 hasta ile kemoradyoterapi uygulanmış 16 hasta çalışmaya dahil edildi. Konstipas-yonun standardize edilmiş şekilde sorgulanması için Roma III kriterleri, yaşam kalitesinin değerlendirilmesi için ise Avrupa Kanser Araştırma ve Tedavi Organizasyonu Yaşam Kalitesi Ölçeği-C30 ve konstipasyon semptom skalası kullanıldı. Bulgular: Yaş ortalaması 60.9 (51-75 yaş aralığı) olan 15 total larenjektomi hastası ve ortalama yaşı 57.6 (48-69 yaş aralığı) olan 16 kemoradyoterapi hastasının verileri incelendi. Roma III kriterleri ile konstipasyon varlığı değerlendirildiğinde, total la-renjektomi grubunda konstipasyon sıklığının, kemoradyoterapi grubuna göre, istatistiksel olarak anlamlı olacak şekilde, daha fazla olduğu göz-lendi (p=0.007). C30 konstipasyon semptom medyan skorları açısından gruplar arasında anlamlı farklılık gözlenmedi. C30 global yaşam kalitesi medyan skorları, kemoradyoterapi grubunda daha yüksek olmasına rağ-men, gruplar arasında istatistiksel olarak anlamlı farklılık bulunmadığı görüldü. Sonuç: Glottik kapanmanın kalıcı olarak kaybolması, kapalı glottis ıkınma fizyolojisini ortadan kaldırarak etkin defekasyonun baş-langıç ve sonlanımı için gerekli itiş gücünü etkilemekte ve fonksiyonel konstipasyona yol açmaktadır. Hali hazırda ileri evre kanser olmaları ne-deniyle artmış konstipasyon riski olan bu hastaların, uygulanan tedavi stratejisine bağlı olarak mevcut riskleri ve konstipasyon sıklığı artmak-tadır. Bu durumu ortaya koymak için, Roma kriterleri gibi standardize sorgulamaların kullanılması çok daha faydalı olacak gibi görünmektedir.

Abstract

Background and Aims: The quality of life decreases in patients after total laryngectomy because of its adverse effects on basic functions, such as breathing, speech, swallowing, and smell. While considering the quality of life in patients, their characteristics of constipation or def-ecation are often overlooked. This study aimed to evaluate the frequen-cy of functional constipation in patients with advanced-stage laryngeal cancer treated with different treatment strategies by using the stan-dardized inquiries and determine its impact on the quality of life. Mate-rials and Methods: In total, 15 and 16 patients who underwent total laryngectomy and chemoradiotherapy, respectively, for advanced-stage laryngeal cancer were included in this study. Rome III criteria were used for the standardized questioning of constipation, and the Euro-pean Organization for Research and Treatment Cancer Quality of Life Questionnaire-C30 and its constipation symptoms scale were used for assessing the quality of life. Results: In total, 15 patients who under-went laryngectomy with a mean age of 60.9 (range 51–75 years) and 16 patients who underwent chemoradiotherapy with a mean age of 57.6 (range 48–69 years) were analyzed. The presence of constipation was evaluated by Rome III criteria, which concluded that the frequen-cy of constipation in the total laryngectomy group was significantly higher than the chemoradiotherapy group (p = 0.007). No significant differences were observed between the groups in terms of C30 median scores for constipation symptom. Although the global quality of life median scores were higher in the chemoradiotherapy group, there was no statistically significant difference between the groups. Conclusion: Permanent disappearance of glottic closure eliminates the closed glot-tis straining physiology and adversely affects the impulse required for the onset and termination of effective defecation, thereby leading to functional constipation. In addition to being at advanced-stage cancer, the applied treatment strategy further increased the risk of constipation in the patients with laryngectomy. Using a standardized questionnaire, such as Rome criteria, would be much more useful to demonstrate this situation.
