Hepatit B taşıyıcı annelerin çocuklarının aşılanması ve takibi

Özet
Amaç: Bu çalışmada son trimester gebelerde hepatit B virus (HBV) seropozitifliği araştırılıp, Hepatit B yüzey antijeni (HBs) pozitif bulunan anneler ve çocukları izlenerek rutin aşılama sonucu çocuklarda oluşan antikor cevabı incelendi. Gereç ve Yöntem: Gaziantep il merkezinde 13 sağlık ocağı bölgesinde 22 farklı sağlık evi seçildi ve 1998-1999 yıllarında bu bölgelerde yaşayan 397 son trimester gebeden elde edilen serumlarda Hepatit B yüzey antijen ve antikoru (AntiHBs) ELISA yöntemi ile çalışıldı. Yaşlan 15-44 arası olan hamileliğin son trimesterindeki toplam 397 gebe kadının doğan çocukları rutin ulusal aşılama programı doğrultusunda yaşamlarının ilk yılında HBV'ye karşı toplam 3 doz aşı ile aşılandı. Aşılama Sağlık Bakanlığının sağladığı rekombinan HBV aşısı ile 2, 3 ve 9. aylarda sağlık ocağı personelleri tarafından uygun şekilde yapıldı. Gebeliğinde HBs pozitif bulunan 37 anne ve doğan çocukları bir ve beş yıl sonra, birlikte tekrar HBs-antiHBs açısından değerlendirildi. Bulgular: Toplam 397 son trimester gebede HBs pozitifliği 37 (%9.3), antiHBs pozitifliği 110 (%27.7) olarak bulundu. Gebeliğinde HBs pozitif bulunan 37 kadın ve doğumdan sonra HBV'ye karşı aşılanmış olan çocukları, aşılamadan bir yıl sonra tekrar değerlendirildi. Ulaşılan toplam 31 anne ve çocuğu HBV açısından tekrar incelendi. Toplam 31 anneden 28'inde (%90.3) HBs pozitif, 3'ünde (%9.7) antiHBs pozitif (spontan serokonversiyon) bulundu. HBs pozitif olan 28 annenin 25'inde (%89.3) AntiHBe pozitif, 3 annede ise (%10.7) HBe pozitifliği vardı. Çocukların 20'sinde (%64.5) anti-HBs, 3 çocukta (%9.7) HBs ve HBe antijenleri pozitif olarak saptandı. Sekiz çocukta ise (%25.8) anti-HBs ve HBs negatif olarak bulundu. Beşinci yıl kontrollerinde toplam 20 çocuk halen anti-HBs pozitif olarak saptandı. Hamilelikte HBs ve HBe pozitif üç anneden sadece birine ulaşılabildi. Bu anne ve aşılanan çocuğun kronik hepatit olduğu saptandı. Sonuç: Hamile olan anne adaylarının HBs pozitifliğinin yanısıra diğer hepatit işaretleyicilerinin de incelenmesi ve doğum sonu antiserum yapılması HBV bulaşını tam olarak önleyecektir.
Abstract
Objective: In this study, HBV seropositivity was searched in women in the third trimester of pregnancy and HBs positive mothers and their children were observed. The antibody response was evaluated following routine immunization. Material and method: In Gaziantep, 397 pregnant women in the 3rd trimester were evaluated for HBs and anti HBs by the enzyme immuno assay (E1A) method between 1998 and 1999. The age of the mothers ranged from 15 to 44. All the children were immunized according to the routine national vaccination programme. Vaccination by recombinant HBV vaccine provided by the Ministry of Health, was administered by health centre staff at the 2nd, 3rd and 9th month post-natal. Thirty seven mothers who were diagnosed as HBs positive and their children were evaluated one and five years later for HBs and antiHBs. Results: From a total of 397 third trimester pregnancies, 37 were HBs positive (9.3%) and 110 were anti-HBs positive (27.7%). The children of the 37 HBs positive women were vaccinnated and evaluated one year later. Of the 31 who could be contacted, 28 (90.3%) were HBs positive and 3 (9.7%)were anti-HBs positive (spontaneous seroconversion). 25 (89.3%) out of 28 mothers who were HBs positive were also positive for antiHBe and 3 (10.7%) for HBe. Of the children, 20 (64.5%) were anti-HBs positive and 3 (9.7%) showed HBs and HBe positive. In 8 children (25.8%) antiHBs and HBs were negative. At the 5-year follow-up, 20 children were still positive for antiHBs. Of 3 mothers who were positive for HBs and HBe when pregnant, only one could be contacted. This mother and her child, who had been vaccinnated, both had chronic hepatitis. Conclusion: When pregnant women are positive for HBs and other markers of hepatitis, antiserum should be administered after birth to prevent the spread of HBV.
