Özet
Asistoli ile gelen hastada başarılı terapotik hipotermi uygulaması: Olgu SunumuKardiyak arrest (KA) sonrası komatöz hastalarda beyin hasarının önlenmesinde terapotik hipotermi (TH) etkilidir. Bu yazıda asistoli ile gelen bir hastada, kardiyopulmoner resüsitasyon (KPR) sonrası başarı ile uygulanan TH sunuldu.Acil serviste, KA sonrası 45 dakika KPR uygulanan ve sonrasında kardiyoloji kliniği tarafından sağ femoral arterden koroner anjiografi ve arterografi uygulanarak dilate kardiyomiyopati tanısı konulan 39 yaşındaki kadın hasta yoğun bakım ünitemize yatırıldı. TH uygulamak için tarafımızca sol femoral venden soğutma kateteri yerleştirildi. Vücut ısısı saatte 0.5?C düşürülmek üzere 32?C'de TH 72 saat süre ile uygulandı. Bilinci 12. günde açılan koopere, yer, zaman, mekan oryantasyonu tam hastada, 16. günde weaning gerçekleştirildi ve ekstübasyon yapıldı. Takip ve tedavi sırasında, TH uygulamasından sonra, koroner anjiografinin uygulandığı sağ femoral arterde psödoanevrizma oluştu ve opere edildi. Sonrasında bu sahada geniş doku defekti gelişti. Hasta yoğun bakım ünitesine kabulünün 31. gününde bilinci açık, tam koopere, oda havasında solur halde sağ femoral bölgedeki doku defektinin tamiri için plastik cerrahi servisine transfer edildi.Bizim olgumuzda görüldüğü gibi TH, KA sonrası komatöz hastalarda nörolojik gelişme sağladığından dikkate alınması gereken bir prosedürdür.
Abstract
Successful therapeutic hypothermia management for a patient with asystole: Case reportTherapeutic hypothermia is efficient on comatose patients after cardiac arrest. This article aims to demonstrate successful therapeutic hypothermia management for a patient with asystole. A 39 years old female patient, to whom after cardiac arrest due to dilated cardiomyopathy 45 minutes CPR in emergency department was performed and an arterial catheter was applied via right femoral artery for coronary for angiograpy and arteriograpy, hospitalized in our intensive care unit. We have inserted a left femoral vein catheter for therapeutic hypothermia application. Therapeutic hypotermia was performed to reduce body temperature 0.5?C per hour until 32?C was reached and continued for 72 hours. The consciousness of the patient opened at 12th day and at at 16th day we applied weaning and extubated the patient. During the observation and treatment period after TH a pseudoaneurysm was developed at the right femoral artery and the patient was operated by the cardiovascular surgery. Afterwards a large tissue defect developed in the operation region. After completed follow-upand treatment in the ICU, the patient was transferred at full conscious and breathing condition to the plastic surgery clinic for tissue defect repairing, on 31th day. Therapeutic hypothermia improves neurological recovery in comatose post-cardiac arrest patients and it should be taken into consideration.
